摘 要:目的 探讨溴吡斯的明联合生物反馈电刺激治疗全子宫切除术后盆底功能障碍的临床疗效,以及对患者转化生长因子β1(TGF-β1)、胰岛素样生长因子1(IGF-1)、基质金属蛋白酶-2(MMP-2)水平及盆底肌电生理指标水平的影响。方法 选取医院2018年6月至2019年6月收治的全子宫切除术后盆底功能障碍患者120例,随机分为观察组和对照组,各60例。两组患者均予生物反馈电刺激治疗,观察组加用溴吡斯的明,均连续治疗4周。结果 观察组总有效率为93.33%,显著高于对照组的78.33%(P <0.05)。治疗后,观察组患者的盆腔脏器脱垂症状(POP-S)、国际尿失禁咨询委员会尿失禁问卷简表(ICI-Q-SF)评分均显著低于对照组,盆底肌力评分显著高于对照组(P <0.05);观察组患者的盆底肌电生理指标Ⅰ类肌纤维、Ⅱ类肌纤维电位均显著高于对照组(P <0.05);观察组患者的TGF-β1,IGF-1,MMP-2水平均显著低于对照组(P <0.05)。治疗期间,观察组和对照组患者的药品不良反应发生率相当(13.33%比8.33%,P> 0.05)。结论 溴吡斯的明联合生物反馈电刺激治疗全子宫切除术后盆底功能障碍临床疗效良好,能改善患者的症状评分、盆底肌电生理指标及血清学指标水平,且安全性良好。Objective To investigate the clinical efficacy of pyridostigmine bromide combined with biofeedback electrical stimulation in the treatment of pelvic floor dysfunction after panhysterectomy,and its effect on the levels of transforming growth factor β1(TGF-β1),insulin-like growth factor 1(IGF-1),matrix metalloproteinase-2(MMP-2) and electrophysiological indexes of pelvic floor muscle of patients.Methods A total of 120 patients with pelvic floor dysfunction after panhysterectomy admitted to the hospital from June 2018 to June 2019 were selected and randomly divided into the observation group and the control group,with 60 cases in each group. The patients in both groups were treated with biofeedback electrical stimulation,on this basis,the patients in the observation group were treated with pyridostigmine bromide. Both groups were treated continuously for four weeks.Results The total effective rate in the observation group was 93. 33%,which was significantly higher than 78. 33% in the control group(P < 0. 05). After treatment,the scores of the Pelvic Organ Prolapse Symptoms(POP-S) and the International Urinary Incontinence Advisory Committee Urinary Incontinence Questionnaire Short Form(ICI-Q-SF) in the observation group were significantly lower than those in the control group,while the score of pelvic floor muscle strength in the observation group was significantly higher than that in the control group(P < 0. 05). After treatment,the potentials of electrophysiological indexes of pelvic floor muscle such as type Ⅰ muscle fiber and type Ⅱ muscle fiber in the observation group were significantly higher than those in the control group(P < 0. 05). After treatment,the levels of TGF-β1,IGF-1 and MMP-2 in the observation group were significantly lower than those in the control group(P < 0. 05).
