摘 要:目的 系统评价810 nm阈值下微脉冲激光(subthreshold diode laser micropulse photocoagulation,SDMP)相较传统激光(conventional laser photocoagulation,CLP)治疗糖尿病黄斑水肿(diabetic macular edema,DME)的疗效。方法 计算机检索PubMed、Embase、Web of Science、Cochrane、SionMed、ClinicalTrials.gov、CNKI数据库和万方数据库,搜索有关810 nm SDMP比较CLP治疗DME的随机对照研究(RCTs),检索时限为2001年12月—2021年12月。结果 本研究共纳入了6篇RCTs,共420眼,Meta分析结果显示:与CLP组相比,810 nm SDMP组在改善最佳矫正视力方面具有更好的疗效(MD=-0.07,95%CI:-0.12~-0.03,P<0.05),且810 nm SDMP组的黄斑中心凹厚度减少比CLP组更为显著,两组之间差异有统计学意义(MD=-8.90,95%CI:-17.22~-0.58,P=0.04)。结论 810 nm SDMP在改善最佳矫正视力、降低黄斑中心凹厚度方面的疗效均优于CLP。Objective To systematically evaluate the efficacy of micropulse laser(SDMP) at 810 nm threshold compared with conventional laser(CLP) in the treatment of diabetic macular edema(DME).Methods Computer searches PubMed,Embase,Web of Science,Cochrane,SionMed,ClinicalTrials.gov,CNKI database and Wanfang database for randomised controlled studies(RCTs) on 810 nm SDMP versus CLP in the treatment of DME,the search time limit was December 2001 to December 2021.Results A total of 6 RCTs with a total of 420 eyes were included in this study.The results of meta-analysis showed that compared with the CLP group,the 810 nm SDMP group had better efficacy in improving the best corrected visual acuity(MD=-0.07,95%CI:-0.12--0.03,P<0.05),and the reduction of foveal thickness in the 810 nm SDMP group was more significant than that in the CLP group,and there was a statistically significant difference between the two groups(MD=-8.90,95%CI:-17.22--0.58,P=0.04).Conclusion 810 nm SDMP is better than CLP in improving best corrected visual acuity and reducing foveal thickness.
