摘 要:目的 以中老年2型糖尿病患者为研究对象,分析其内脏脂肪、代谢指标对骨密度的影响。方法 选取2020年1月—2022年1月在常州市中医医院住院的2型糖尿病患者245例为研究对象。测量患者腰椎骨密度与内脏脂肪面积,并按照骨密度平均值分为两组,骨量正常组患者98例、骨量异常组患者147例,比较两组性别、平均年龄、糖尿病病程、BMI指数、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、尿酸(UA)、血钙(Ca)、血磷(P)、β胶原降解产物(β-CTX)、甲状旁腺激素(PTH)、C反应蛋白(CRP)、内脏脂肪面积(VFA)。结果 骨量异常组的性别、平均年龄、糖尿病病程、HbA1c、TG、TC、UA、PTH、β-CTX、CRP、VFA与骨量正常组比较,差异有统计学意义(P<0.05)。性别、平均年龄、糖尿病病程、UA、CRP、VFA是导致2型糖尿病患者骨量异常的独立危险因素(P<0.05)。结论 2型糖尿病患者的内脏脂肪与代谢指标会对骨密度造成影响,增加骨量异常发生率。Objective To study the effects of visceral fat,metabolic indexes on bone mineral density in middle-aged and elderly patients with type 2 diabetes mellitus.Methods A total of 245 patients with type 2 diabetes who were hospitalized in the Changzhou Hospital of Traditional Chinese Medicine from from January 2020 to January 2022 were selected as the research subjects.The lumbar spine bone mineral density and visceral fat area were measured,and the patients were divided into two groups according to the average bone mineral density.There were 98 patients in the normal bone mass group and 147 patients in the abnormal bone mass group.The gender,average age,duration of diabetes,BMI,fasting blood glucose(FPG),glycosylated hemoglobin(HbA1c),total cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),High-density lipoprotein cholesterol(HDL-C),uric acid(UA),blood calcium(Ca),blood phosphorus(P),β-collagen degradation products(β-CTX),parathyroid hormone(PTH),C-reactive protein(CRP),Visceral fat area(VFA) and compared.Results The gender,mean age,diabetes duration of the abnormal bone mass group,HbA1c,TG,TC,UA,PTH,β-CTX,CRP,and VFA area were compared with the normal bone mass group,the difference was statistically significant(P<0.05).Gender,average age,duration of diabetes,UA,CRP,and VFA were independent risk factors for abnormal bone mass in patients with type 2 diabetes(P<0.05).Conclusion Visceral fat and metabolic indexes in patients with type 2 diabetes can affect bone mineral density and increase the incidence of abnormal bone mass.
