摘 要:目的 探究在糖尿病高渗性昏迷患者干预中应用胰岛素泵的临床效果。方法 以2019年2月—2021年11月福建省立金山医院收治的110例糖尿病高渗性昏迷患者为例,随机将患者分为两组,每组55例,在给予患者急救干预的过程中,给予不同胰岛素注射方法,分别为多次皮下胰岛素注射(对照组)和胰岛素泵下的持续胰岛素注射(研究组),对两组患者治疗前后的血糖水平、急救有效率和胰岛素使用剂量、血糖水平达标时间等效果进行比较。结果 研究组患者的空腹血糖值和糖化血红蛋白值分别为(6.66±1.23)mmol/L和(6.74±1.64)%,均比对照组低,差异有统计学意义(t=2.315、3.188,P<0.05)。研究组患者的胰岛素使用剂量和血糖达标时间分别为(60.11±9.44)U和(2.15±1.33)d,均比对照组少,差异有统计学意义(t=5.606、2.480,P<0.05)。研究组中治疗有效率为96.36%,高于对照组的83.64%,差异有统计学意义(χ2=4.949,P<0.05)。结论 在糖尿病高渗性昏迷患者干预过程中应用胰岛素泵,可促进患者恢复,将患者的血糖水平控制在稳定范围内,避免对患者治疗造成不良影响,临床效果较好。Objective To explore the clinical effect of insulin pump application in the intervention of diabetic hyperosmolar coma patients.Methods Taking 110 patients with diabetic hyperosmolar coma selected in Fujian Provincial Jinshan Hospital as an example,the study period was from February 2019 to November 2021.The patients were randomly divided into two groups,55 cases in each group,and different insulin injection methods were given during the emergency intervention.Multiple subcutaneous insulin injections(control group) and continuous insulin injections under an insulin pump(study group),respectively.The blood glucose levels,emergency response rates,insulin doses,and time to reach the target blood glucose levels were observed and compared in the two groups before and after treatment.Results The fasting blood glucose value and glycated hemoglobin value of the study group were(6.66±1.23)mmol/L and(6.74±1.64) %,which were lower than those of the control group,and the differences were statistically significant(t=2.315,3.188,P<0.05).The insulin dosage and blood glucose target time of the study group were(60.11±9.44) U and(2.15±1.33) d,respectively,which were lower than those of the control group,and the differences were statistically significant(t=5.606,2.480,P<0.05).The effective rate of the study group was 96.36%,which was higher than that of the control group(83.64%),and the difference was statistically significant(χ^(2)=4.949,P<0.05).Conclusion The application of insulin pump in the intervention process of diabetic hyperosmolar coma patients can promote the recovery of patients,control the blood glucose level of patients in a stable range,and avoid adverse effects on the treatment of patients,and the clinical effect is good.
