摘　　要：目的 探讨不同超滤率及超滤量对老年糖尿病肾病血液透析患者代谢功能的影响。方法2020年12月—2021年12月选择于绥化市第一医院诊疗且符合研究标准的78例老年糖尿病肾病血液透析患者为研究对象,应用随机数表法划分成对照组(n=39)、观察组(n=39)。对照组在血液透析过程中采取高超滤率和高超滤量治疗,观察组在血液透析过程中采取正常超滤率和超滤量治疗,评价各组患者治疗效果、相关指标、血脂代谢指标、炎症反应指标、肾功能。结果 观察组治疗总有效率为92.31%,明显高于对照组的74.36%,差异有统计学意义(P<0.05);观察组治疗后血压水平均明显优于对照组,差异有统计学意义(P<0.05);观察组治疗后脂水平均明显低于对照组,差异有统计学意义(P<0.05);观察组治疗后炎性因子水平均显著低于对照组,差异有统计学意义(P<0.05);观察组治疗后肾功能指标均显著较对照组优,差异有统计学意义(P<0.05)。结论 老年糖尿病肾病患者采取正常超滤率和超滤量进行血液透析治疗更具有效果,可明显改善机体代谢功能、炎症反应指标以及肾功能,血压波动水平较小,可获取更为理想的疗效,对减轻疾病损害和改善预后具有重要意义,值得临床推广。Objective To explore the effect of different ultrafiltration rate and ultrafiltration volume on metabolic function of elderly diabetic nephropathy patients on hemodialysis.Methods A total of 78 elderly diabetic nephropathy hemodialysis patients who were diagnosed and treated in Suihua First Hospital of Suihua City and who met the research criteria from December 2020 to December 2021 were selected as the subject of inquiry.The patients were divided into control group(n=39) and observation group(n=39) by random number table method.The control group was treated with high ultrafiltration rate and high ultrafiltration volume during hemodialysis,while the observation group was treated with normal ultrafiltration rate and ultrafiltration volume during hemodialysis.The treatment effect,related indicators,blood lipid metabolism indicators,inflammatory response indicators,and renal function were evaluated in each group.Results The total effective rate of the observation group was 92.31%,which was significantly higher than that of the control group74.36%,the difference was statistically significant(P<0.05);after treatment,the blood pressure levels in the observation group were significantly better than those in the control group,the difference was statistically significant(P<0.05);after treatment,the blood lipid levels in the observation group were significantly lower than those in the control group,the difference was statistically significant(P<0.05);after treatment,the inflammatory factor levels in the observation group were significantly lower than the control group,the difference was statistically significant(P<0.05);after treatment,the kidney tunction indicators in the observation group were significantly lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion Hemodialysis with normal ultrafiltration rate and ultrafiltration volume is more effective in elderly diabetic nephropathy patients,which can significantly improve the body’s metabolic function,inflammatory response
