摘 要:目的 分析尿糖、尿微量白蛋白联合检验应用于糖尿病早期肾损伤诊断中的效果及准确率。方法 选取厦门大学附属中山医院2021年1—12月收治的糖尿病早期肾损伤的60例患者为损伤组,并选取糖尿病(diabetes mellitus,DM)未发生肾损伤患者60例为DM组,另选取同期本院接受健康体检者60名为对照组。比较3组血糖指标、尿糖、尿微量白蛋白及联合检测阳性率。结果 损伤组与DM组血糖指标及DM组与对照组尿微量白蛋白阳性率相比,差异无统计学意义(P>0.05)。损伤组与DM组血糖指标高于对照组,差异有统计学意义(P<0.05),且损伤组与DM组患者尿糖及联合检测阳性率均高于对照组,差异有统计学意义(P<0.05),且损伤组尿糖、尿微量白蛋白及联合检测阳性率均高于DM组,差异有统计学意义(P<0.05)。结论 在糖尿病早期肾损伤诊断中,以血糖指标水平并不能明确诊断糖尿病与糖尿病早期肾损伤,且单一尿糖或尿微量白蛋白检测的诊断率也并不理想。而以尿糖、尿微量白蛋白联合检验的方式,可以明显提高对糖尿病早期肾损伤的诊断准确率。Objective To analyze the effect and accuracy of the combined test of urine glucose and urine microalbumin in the diagnosis of early diabetic kidney injury.Methods A total of 60 patients who were diagnosed with early diabetic kidney injury in Zhongshan Hospital Affiliated to Xiamen University from January to December 2021 were selected as the injury group,and 60 patients with diabetes mellitus(DM) without renal injury were selected as the DM group.Another 60 people who received health checkup in the hospital during the same period were selected as the control group.The positive rates of blood glucose indexes,urine sugar,urine microalbumin and combined detection were compared among the three groups.Results There was no statisticlly significant difference in the blood glucose index between the injury group and the DM group and the positive rate of urine microalbumin between the DM group and the control group(P>0.05).The blood glucose indexes in the injury group and DM group were higher than those in the control group,the difference was statistically significant(P<0.05),and the positive rates of urine glucose and combined detection in the injury group and DM group were higher than those in the control group,the difference was statistically significant(P<0.05).The positive rates of urine glucose,urine microalbumin and combined detection in the injury group were higher than those in the DM group,and the difference was statistically significant(P<0.05).Conclusion In the diagnosis of early diabetic kidney injury,the level of blood glucose indicators cannot make a clear diagnosis of diabetes and early diabetic kidney injury,and the diagnostic rate of single urine glucose or urine microalbumin detection is not ideal.The combined test of urine glucose and urine microalbumin can significantly improve the diagnostic accuracy of early diabetic kidney injury.
