GADA、IA-2A、ZnT8A联合IGRPA检测在1型糖尿病诊断中应用研究

摘　　要：目的 探讨谷氨酸脱羧酶抗体(GADA)、蛋白质酪氨酸磷酸酶抗体(IA-2A)、锌转运体8抗体(ZnT8A)联合胰岛特异的葡萄糖-6-磷酸酶催化亚基相关蛋白抗体(IGRPA)在1型糖尿病(type 1 diabetes,T1DM)中的诊断价值。方法 选取2019年6月—2021年6月大庆龙南医院收治的43例T1DM患者纳为T1DM组,同期于本院行正常体检的48名健康体检者为健康对照组。检测两组GADA、IA-2A、ZnT8A与IGRPA,分析比较两组上述指标检测的检出阳性率;同时绘制受试者相关曲线(ROC),分析GADA、IA-2A、ZnT8A与IGRPA单独及联合检测T1DM的诊断效能。结果 T1DM组的GADA、IA-2A、ZnT8A与IGRPA阳性率[65.12%(28/43)、46.51%(20/43)、79.07%(34/43)、76.74%(33/42)]均高于与健康对照组,差异有统计学意义(χ~2=34.992、22.186、49.849、50.478,P<0.05);ROC曲线显示:四种抗体联合检测的AUC为0.900(95%CI:0.828~0.972),高于四种抗体单独检测。结论 GADA、IA-2A、ZnT8A联合IGRPA检测T1DM效果显著,能够明显提高疾病检出率,值得临床大力推行。Objective Investigate glutamate decarboxylase antibody(GADA),protein tyrosine phosphatase antibody(IA-2 A),zinc transporter 8 antibody(ZnT8 A) combined with islet-specific glucose-6-phosphatase catalytic subunit-related protein antibody(IGRPA) in the diagnosis of type 1 diabetes mellitus(type 1 diabetes,T1 DM).Methods A total of 43 patients with T1 DM admitted to the hospital from June 2019 to June 2021 were included in the T1 DM group.The 48 healthy subjects who underwent normal physical examination in our hospital during the same period were the healthy control group.Two groups of 5 mL venous blood were collected,and their GAD A,IA-2 A,ZnT8 A and IGRPA were detected by enzyme-linked immunosorbent assay.The positive rates of the two groups of the above-mentioned indicators were analyzed and compared.At the same time,the subject correlation curve(ROC) was drawn to analyze the diagnostic efficacy of GAD A,IA-2 A,ZnT8 A and IGRPA alone and in combination to detect T1 DM.Results The positive rates of GAD A,IA-2 A,ZnT8 A and IGRPA in the T1 DM group were [65.12%(28/43),46.51%(20/43),79.07%(34/43),76.74%(33/42)],higher than healthy control group,the difference was statistically significant(χ~2=42.769,29.120,66.669,69.047,P<0.05).The ROC curve showed that the AUC of the combined detection of the four antibodies was 0.900(95%CI:0.828-0.972),which was higher than that of the four antibodies alone.Conclusion GADA,IA-2 A,ZnT8 A combined with IGRPA has a significant effect in the detection of T1 DM,and can significantly improve the detection rate of the disease,which is worthy of vigorous clinical promotion.
