摘　　要：目的 研究蚕食清疮换药法用于气虚血瘀证糖尿病足溃疡患者护理的临床疗效。方法 选取2018年1月—2019年10月首都医科大学附属北京中医医院收治的气虚血瘀证糖尿病足溃疡患者60例,按随机数字表法分为两组。对照组给予常规外科清创换药法处理伤口;观察组给予蚕食清疮换药法处理创面。观察记录两组患者的创面面积、疼痛评分。结果 两组创面面积均较治疗前缩小,且观察组创面面积小于对照组,差异有统计学意义(P<0.05);两组疼痛评分均较治疗前降低,且观察组疼痛评分低于对照组,差异有统计学意义(P<0.05)。结论 采用蚕食清疮换药法处理气虚血瘀证糖尿病足溃疡在创面面积及疼痛评分方面效果优于常规外科清创换药,且此项操作可由专科护士独立完成,在应用中未发生不良事件,值得临床进一步推广。Objective To study the clinical effect of nibbling clearing sore and changing dressings in nursing of patients with diabetic foot ulcer with qi deficiency and blood stasis syndrome.Methods A total of 60 patients with diabetic foot ulcer with qi deficiency and blood stasis syndrome in the Beijing Hospital of Traditional Chinese Medicine,Capital Medical University from January 2018 to October 2019 were selected.The patients were divided into two groups according to the random number table method.The control group was given routine surgical debridement and dressing change to treat the wound,and the observation group was given the cannibalization debridement and dressing change method to treat the sore surface.The wound area and pain score of the two groups were observed and recorded.Results The wound area of the two groups was smaller than before treatment,and the wound area of the observation group was smaller than that of the control group,and the difference was statistically significant(P<0.05).The pain scores of the two groups were lower than before treatment,and the pain scores of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The effect of nibbling,clearing sores and changing dressings in treating diabetic foot ulcers with qi deficiency and blood stasis syndrome is better than conventional surgical debridement and dressing change in terms of wound area and pain score.Moreover,this operation can be completed independently by specialist nurses,and no adverse events have occurred in the application,which is worthy of further promotion.
