空腹血糖、糖化血红蛋白及血清C肽水平在2型糖尿病伴视网膜病变患者中的变化情况及临床意义

摘　　要：目的 研究空腹血糖(FBG)、糖化血红蛋白(HbA1c)以及血清C肽水平在2型糖尿病伴视网膜病变患者中的变化情况及临床意义,为临床早期诊治糖尿病视网膜病变(DR)提供理论依据。方法 选取2020年1月至2021年8月我院收治的100例2型糖尿病患者作为研究对象,根据患者是否伴有DR将其分为有DR组(n=50)和无DR组(n=50);另选取同期来我院体检的50例健康志愿者作为对照组。检测并比较三组研究对象的FBG、HbA1c以及血清C肽水平;再根据眼底病变程度将有DR组患者分为Ⅰ、Ⅱ、Ⅲ、Ⅳ期,比较不同程度眼底病变患者的FBG、HbA1c以及血清C肽水平;分析FBG、HbA1c以及血清C肽水平与2型糖尿病合并视网膜病变程度的相关性。结果 三组研究对象的FBG、HbA1c以及血清C肽水平比较,差异均有统计学意义(P<0.05);有DR组的FBG水平及HbA1c均显著高于无DR组,无DR组显著高于对照组,差异具有统计学意义(P<0.05);有DR组的血清C肽水平显著低于无DR组,无DR组显著低于对照组,差异具有统计学意义(P<0.05)。不同程度眼底病变患者的FBG、HbA1c以及血清C肽水平比较,差异均有统计学意义(P<0.05);Ⅰ期患者的FBG水平及HbA1c显著低于Ⅱ期,Ⅱ期显著低于Ⅲ期,Ⅲ期显著低于Ⅳ期,差异具有统计学意义(P<0.05);Ⅰ期患者的血清C肽水平显著高于Ⅱ期,Ⅱ期显著高于Ⅲ期,Ⅲ期显著高于Ⅳ期,差异具有统计学意义(P<0.05)。Pearson相关性分析结果显示,FBG水平以及HbA1c与2型糖尿病合并视网膜病变程度呈正相关,血清C肽水平与2型糖尿病合并视网膜病变程度呈负相关(P<0.05)。结论 FBG、HbA1c以及血清C肽水平与2型糖尿病伴视网膜病变患者眼底病变程度有密切的关系,检测FBG、HbA1c以及血清C肽水平能判断2型糖尿病患者的视网膜病变程度,对评估患者预后起到重要作用。Objective To study the changes and clinical significance of fasting blood glucose(FBG),hemoglobin A1c(HbA1c) and serum C-peptide levels in type 2 diabetes mellitus patients with retinopathy,and provide theoretical basis for early diagnosis and treatment of diabetic retinopathy(DR) in clinic.Methods A total of 100 type 2 diabetes mellitus patients treated in our hospital from January 2020 to August 2021 were selected as the research objects.According to whether the patients accompanied with DR,the patients were divided into DR group(n=50) and no-DR group(n=50);in addition,50 healthy volunteers who came to our hospital for physical examination in the same period were selected as control group.The levels of FBG,HbA1c and serum C-peptide in the three groups were detected and compared;according to the degree of fundus lesions,the patients in the DR group were divided into stage Ⅰ,Ⅱ,Ⅲ and Ⅳ,and the levels of FBG,HbA1c and serum C-peptide in patients with different degrees of fundus lesions were compared;the correlation between the levels of FBG,HbA1c,serum C peptide and the degree of retinopathy in type 2 diabetes mellitus were analyzed.Results The levels of FBG,HbA1c and serum C-peptide in the three groups were significantly different(P<0.05);the level of FBG and HbA1c in the DR group were significantly higher than those in the no-DR group,and those in the no-DR group were significantly higher than the control group,the differences were statistically significant( P<0.05);the serum C-peptide level in the DR group was significantly lower than that in the no-DR group,and that in the no-DR group was significantly lower than the control group,the differences were statistically significant(P<0.05).The levels of FBG,HbA1c and serum C-peptide with different degree of fundus lesions were significantly different(P <0.05);the level of FBG and HbA1c in patients with stage Ⅰ were significantly lower than stage Ⅱ,those in patients with stage Ⅱ were significantly lower than stage Ⅲ,those in patients with stage Ⅲ we
