股骨外侧单皮质重建锁定钢板术联合PFNA对股骨粗隆间粉碎性骨折患者手术相关指标及关节功能的影响

摘　　要：目的 探究股骨外侧单皮质重建锁定钢板术联合防旋股骨近端髓内钉(PFNA)对股骨粗隆间粉碎性骨折患者手术指标及髋关节功能的影响。方法 选取2018年1月至2020年12月我院收治的60例股骨粗隆间粉碎性骨折患者作为研究对象,根据数字标注法的原则将其分为对照组和研究组,各30例。对照组仅采取PFNA治疗,研究组采取股骨外侧单皮质重建锁定钢板术联合PFNA治疗。比较两组的临床效果。结果 研究组的手术时间、术后卧床时间及骨折愈合时间均短于对照组,术后引流量少于对照组(P<0.05)。术后,两组的数字评分法(NRS)评分低于术前,Harris髋关节功能(HHS)、改良Barthel指数(MBI)及世界卫生组织生活质量简易量表(WHOQOL-BREF)评分均高于术前,且研究组优于对照组(P<0.05)。术后3个月,研究组的关节功能恢复优良率为70.00%,显著高于对照组的50.00%(P<0.05);术后6个月,两组的关节功能恢复优良率无明显差异(P>0.05)。术后,两组患者的红细胞压积(HCT)、血红蛋白(Hb)水平均低于术前,但研究组高于对照组(P<0.05)。研究组的并发症总发生率低于对照组(P<0.05)。研究组的满意度高于对照组(P<0.05)。结论 股骨外侧单皮质重建锁定钢板术联合PFNA治疗股骨粗隆间粉碎性骨折的效果显著,可促进患者关节功能恢复及骨折愈合,缩短手术用时,降低并发症发生率。Objective To explore the effects of lateral femoral monocortex reconstruction locking plate combined with proximal femoral nail antirotation(PFNA) on operation-related indicators and joint function of patients with intertrochanteric comminuted fracture.Methods A total of 60 patients with intertrochanteric comminuted fracture treated in our hospital from January 2018 to December 2020 were selected as the research objects,and the patients were divided into control group and study group according to the principle of digital labeling method,with 30 cases in each group.The control group was treated with PFNA,and the study group was treated with lateral femoral monocortex reconstruction locking plate combined with PFNA.The clinical effects of the two groups were compared.Results The operation time,postoperative bed rest time and fracture healing time of the study group were shorter than those of the control group,and the postoperative drainage volume was less than that of the control group(P0.05).After operation,the levels of hematocrit(HCT) and hemoglobin(Hb) in the two groups were lower than those before operation,but those in the study group were higher than the control group(P<0.05).The total incidence of complications in the study group was lower than that in the control group(P<0.05).The satisfaction of the study group was higher than that of the control group(P <0.05).Conclusion Lateral femoral monocortex reconstruction locking plate combined with PFNA in the treatment of intertrochanteric comminuted fracture has a significant effect,it can promote the recovery of joint function and fracture healing of patients,shorten the operation time,and reduce the incidence of complications.
