联合血管切除重建在胰腺癌根治术中的应用效果及对不良反应发生情况的影响

摘　　要：目的 探讨联合血管切除重建在胰腺癌根治术中的应用效果。方法 选择2018年7月至2020年6月我院肿瘤科收治的120例胰腺癌患者作为研究对象,按照抽签法将其分为对照组和观察组,各60例。对照组应用常规胰腺癌根治术治疗,观察组在对照组基础上联合血管切除重建。比较两组的治疗效果。结果 观察组的手术持续时间长于对照组,术中出血量多于对照组(P<0.05);两组的住院时间比较,差异无统计学意义(P>0.05)。观察组的治疗总有效率、生存率高于对照组(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。术后1个月,观察组的躯体功能、心理功能、社会功能及物质功能评分高于对照组(P<0.05)。结论 联合血管切除重建在胰腺癌根治术中的应用效果显著,虽然延长了手术时间,增加术中出血量,但提高了治疗总有效率、生存率及生活质量,且未增加不良反应。Objective To explore the application effect of combined vascular resection and reconstruction in radical resection of pancreatic cancer.Methods A total of 120 patients with pancreatic cancer admitted in the oncology department of our hospital from July 2018 to June 2020 were selected as the research objects,and the patients were divided into control group and observation group according to the lottery method,with 60 cases in each group.The therapeutic effects were compared between the two groups.Results The operation duration of the observation group was longer than that of the control group,and the intraoperative blood loss was more than that of the control group(P0.05).The total effective rate and survival rate of the observation group were higher than those of the control group(P 0.05).One month after operation,the scores of physical function,psychological function,social function and material function in the observation group were higher than those in the control group(P<0.05).Conclusion Combined vascular resection and reconstruction in radical resection of pancreatic cancer has remarkable application effects.Although the operation time is prolonged and the intraoperative blood loss is increased,but the total effective rate treatment,survival rate and quality of life are improved,and no adverse reactions are increased.
