基于ERAS理念的全程症状管理护理在肝叶切除患者中的应用效果及对术后康复的影响

摘　　要：目的 探讨基于快速康复外科(ERAS)理念的全程症状管理护理在肝叶切除患者中的应用效果及对术后康复的影响。方法 将2020年1月至2021年1月在本院行肝叶切除的76例患者按照电脑数字随机表法分为对照组和观察组,每组38例。对照组予以常规护理,观察组实施基于ERAS理念的全程症状管理护理。比较两组的护理效果。结果护理后,两组的症状发生情况少于护理前,频繁程度、严重程度、困扰程度评分均低于护理前,且观察组优于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。术后7 d,两组的总胆红素(TBIL)、丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)水平均低于术后1 d,且观察组低于对照组(P<0.05)。结论 基于ERAS理念的全程症状管理护理应用于肝叶切除患者中的效果显著。Objective To explore the application effect of whole-course symptom management nursing based on enhanced recovery after surgery(ERAS) concept in patients with hepatectomy and its influence on postoperative rehabilitation.Methods A total of 76 patients who underwent hepatectomy in our hospital from January 2020 to January 2021were divided into control group and observation group according to the computer number random table method,with 38 cases in each group.The control group was given routine nursing,while the observation group was given whole-course symptom management nursing based on ERAS concept.The nursing effects of the two groups were compared.Results After nursing,the occurrence of symptoms in the two groups was less than that before nursing,the scores of frequency,severity and distress were lower than those before nursing,and those in the observation group were better than the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P <0.05).At 7 d after operation,the levels of total bilirubin(TBIL),alanine aminotransferase(ALT) and aspartate transaminase(AST) in the two groups were lower than those at 1 d after operation,and those in the observation group were lower than the control group(P <0.05).Conclusion The whole-course symptom management nursing based on ERAS concept applied in the patients with hepatectomy has a significant effect.
