摘　　要：目的:探讨自拟赤芍汤洗剂治疗痔疮术后切口水肿的临床效果。方法:选择2020年6月—2021年9月收治的痔疮患者80例,按随机数字表法分为对照组和观察组,每组40例。两组均行痔疮外剥内扎术,术后对照组采用高锰酸钾溶液坐浴治疗,观察组采用赤芍汤洗剂治疗,两组均治疗14 d,术后随访5个月。比较两组临床疗效、水肿消退时间、切口水肿评分、切口疼痛、不良反应及复发情况。结果:观察组治疗总有效率(RR)高于对照组,水肿消退时间短于对照组,术后7 d及14 d切口水肿评分、视觉模拟评分法(VAS)评分均低于对照组,差异均有统计学意义(P<0.05);两组治疗过程中均未出现不良反应;两组术后1个月、3个月、5个月复发率比较,差异无统计学意义(P>0.05)。结论:自拟赤芍汤洗剂治疗痔疮术后切口水肿有利于促进水肿消散,减轻患者痛苦,加快创面愈合,且不良反应少、复发率低,临床应用安全、有效。Objective:Clinical observation of self-made Chishao Decoction lotion in the treatment of incision edema after hemorrhoid surgery:a randomized controlled trial.Methods:All 80 patients with hemorrhoids who were treated in Jiujiang Traditional Chinese Medicine Hospital from June 2020 to September 2021 were selected and divided into two groups according to the random number table method,with 40 cases in each group.The two groups were treated with external stripping and internal ligation of hemorrhoids.After operation,the control group was treated with potassium permanganate tablets mixed with water and a sitz bath,and the observation group was treated with Chishao Decoction lotion for 14 days.The patients were followed up for 5 months.The clinical efficacy,edema subsidence time,incision edema score,incision pain,adverse reactions and recurrence were compared between the two groups.Results:The total effective rate(RR)of the observation group was higher than that of the control group,the edema subsidence time was shorter than that of the control group,and the incision edema score at 7 d and 14 d after operation,and visual analogue scale(VAS)scores were lower than those in the control group,with statistical difference(P<0.05).There were no adverse reactions during the process,there was no significant difference in the recurrence rate between the two groups at 1 month,3 months and 5 months after operation(P>0.05).Conclusion:The self-made Chishao decoction lotion in the treatment of incision edema after hemorrhoid surgery is beneficial to promote the dissipation of incision edema,relieve the pain of patients,and accelerate wound healing.It has fewer adverse reactions and low recurrence rate.
