口腔种植手术应用不翻瓣微创法对其种植体稳定程度的影响

摘　　要：目的探讨口腔种植手术应用不翻瓣微创法对其种植体稳定程度的影响,以为临床寻找出更有效的治疗方案。方法选取重庆市开州区人民医院2020年3月—2021年3月收治的102例进行口腔种植治疗的患者,依据随机数表法分为对照组(51例)与研究组(51例)。对照组给予传统翻瓣术治疗,研究组给予不翻瓣微创法治疗。比较两组患者手术相关指标,种植体稳定程度(术后3个月种植体稳定系数、术后6个月牙槽嵴顶骨吸收量),术前及术后3个月满意度评分,种植成功率及术后6个月内不良事件发生情况。结果研究组患者手术相关指标(手术时间、术后抗生素应用时间、术后疼痛反应时间、术后激素使用时间)时间均短于对照组,差异有统计学意义(t=14.882、37.038、29.185、40.882,P<0.05);两组患者术后3个月美观效果、舒适度评分均增加,其中研究组增加幅度明显大于对照组,差异有统计学意义(t=13.602、24.570,P<0.05);研究组患者种植成功率高于对照组,差异有统计学意义(χ^(2)=3.991,P<0.05);术后6个月研究组患者的牙槽嵴顶骨吸收量(0.21±0.05)mm低于对照组(0.88±0.10)mm,差异有统计学意义(t=42.796,P<0.05)。结论口腔种植手术应用不翻瓣微创法可有效缓解患者的临床症状,优化手术相关指标,降低种植体周围骨吸收,促进种植体稳定,提高种植成功率及患者治疗满意度,且安全性高。Objective To explore the effect of non-flap minimally invasive method on implant stability in oral implant surgery,in order to find a more effective clinical treatment plan.Methods A total of 102 patients admitted to the hospital from March 2020 to March 2021 were selected and divided into control group(51 cases)and study group(51 cases)according to random number table method.The control group was treated with traditional valvectomy,and the study group was treated with minimally invasive method without valvectomy.Surgical indicators,implant stability(implant stability coefficient 3 months after surgery,implant absorption 6 months after surgery),preoperative and postoperative satisfaction score 3 months after surgery,implant success rate and adverse events 6 months after surgery were compared between the two groups.Results The time of operation related indexes(operation time,postoperative antibiotic application time,postoperative pain reaction time,postoperative hormone use time)in study group was shorter than that in control group,and the difference was statistically significant(t=14.882,37.038,29.185,40.882,P<0.05).Compared with aesthetic effect and comfort score of 2 groups increased 3 months after surgery,and the increase rate of study group was significantly greater than that of control group,the difference was statistically significant(t=13.602,24.570,P<0.05).The success rate of implantation in study group was higher than that in control group,and the difference was statistically significant(χ^(2)=3.991,P<0.05).6 months after surgery,the absorption amount of alveolar crest and parietal bone in the study group(0.21±0.05)mm was lower than that in the control group(0.88±0.10)mm,the difference was statistically significant(t=42.796,P<0.05).Conclusion The application of nonflap minimally invasive method in oral implant surgery can effectively relieve the clinical symptoms of patients,optimize operation-related indicators,reduce peri-implant bone resorption,promote implant stability,improve implant success rate
