磁共振联合超声检查对输卵管系膜囊肿的诊断价值分析

摘　　要：目的:对输卵管系膜囊肿的磁共振(MRI)及超声检查诊断价值进行探讨。方法:选取南京市妇幼保健院2020年5月—2022年3月经临床手术并术后病理确诊为输卵管系膜囊肿的29例患者,共41个病灶的影像学资料。对比患者影像学资料,分析MRI和超声两者影像学征象的差异性表现。结果:29例患者中22例患者因发现盆腔包块就诊,4例患者因月经紊乱就诊;术中发现合并盆腔粘连15例,合并子宫肌瘤7例,合并恶性肿瘤或癌前病变5例;合并腺肌病或内异症4例。两种检查方法可检测出的输卵管系膜囊肿大小有显著差异(P<0.05);两种检查方法对卵巢系膜囊肿检测的准确性有显著差异(P<0.05)。结论:MRI能发现超声所不能显示的更小病灶,且MRI能更清晰显示囊肿与周围组织的关系,对初步判定包块性质的诊断准确率更高。Objective To investigate the diagnostic value of magnetic resonance(MRI)and ultrasound in tubal mesenteric cysts.Methods The imaging data of 41 lesions in 29 patients with mesosalpinx cyst diagnosed by clinical operation and pathology in Nanjing Maternity and Child Health from May 2020 to March 2022 were selected.The imaging data of the patients were compared and the differences between MRI and ultrasound were analyzed.Results Twenty two patients of the 29 patients visited the hospital because of a discovered pelvic mass,and four patients visited the hospital because of menstrual disturbance;Intraoperative findings were combined pelvic adhesions in 15 cases,combined uterine fibroids in 7 cases,and combined malignancy or precancer in 5 cases;Combined adenomyosis or endometriosis in 4 patients.There was significant difference in the size of mesosalpinx cyst detected by the two methods(P<0.05);There was difference between the two methods in the accuracy of detection of ovarian mesangial cysts.The accuracy of detection of ovarian mesangial cysts difference between the two examination methods was significant(P<0.05).Conclusion MRI detects smaller lesions that cannot be visualized by ultrasound,and MRI provides a clearer visualization of the relationship of the cyst to the surrounding tissue,resulting in a higher diagnostic accuracy for the initial determination of the nature of the mass.
