摘　　要：目的:对比研究早孕期胚胎存活组和停止发育组母体子宫动脉体支血流参数的不同改变,探讨子宫动脉体支血流参数对早孕期胚胎发育不同预后的诊断价值。方法:选取2020年9月—2021年10月于白银市第一人民医院就诊的293例早期妊娠妇女进行经腹超声检查,应用彩色及频谱多普勒对母体双侧子宫动脉体支(宫颈与宫体分界以上1~2 cm处)的S/D、PI、RI进行测量,其中胚胎存活组235例,胚胎停止发育组58例,采用频谱自动包络功能,至少包络4~5个心动周期进行测量,取其平均值并比较两组血流参数S/D、PI、RI值的不同改变。结果:胚胎存活组右侧、左侧及双侧子宫动脉均值S/D、PI、RI均高于胚胎停止发育组,差异有统计学意义(P <0.05)。在两组中,左侧子宫动脉体支S/D、PI、RI各数值均高于右侧。妊娠后血流参数S/D、PI、RI值随着孕期增大而呈现出逐渐降低的趋势,其在停止发育组中尤为明显。结论:子宫动脉体支血流参数S/D、PI、RI对提前评估早孕期胚胎发育不同预后具有重要的临床价值。Objective To investigate the diagnostic value of blood flow parameters of uterine artery body branches for different prognosis of embryo development in early pregnancy,the different changes of blood flow parameters of uterine artery body branches in embryo survival group and embryo cessation group were compared.Methods 293 cases of early pregnant women in Baiyin First People's Hospital from September 2020 to October 2021 were selected for transabdominal ultrasound examination,and the S/D,PI and RI of bilateral uterine artery branches(1~2 cm above the cervix and uterine body boundary)were measured by color and spectral Doppler.In the 58 cases of embryo cessation group,spectral automatic envelope function was used to measure at least 4~5 cardiac cycles.The mean value was taken and the changes of S/D,PI and RI values of blood flow parameters were compared between the two groups.Results The mean S/D,PI and RI of the right,left and bilateral uterine arteries in the embryo survival group were higher than those in the embryo cessation group,and there were significant differences between the two groups(P<0.05).In both groups,S/D,PI and RI values of the body branch of the left uterine artery were higher than those of the right.The blood flow parameters S/D,PI and RI showed a decreasing trend with the increase of pregnancy,especially in the embryo cessation group.Conclusion Uterine artery body branch blood flow parameters S/D,PI,RI have important clinical value for early evaluation of different prognosis of embryo development in early pregnancy.
