彩色多普勒超声心动图诊断成人先天性心脏病的应用价值

摘　　要：目的:分析彩色多普勒超声心动图诊断成人先天性心脏病的应用价值。方法:选取2019年8月—2021年8月期间在日照市岚山区人民医院诊疗的疑似成人先天性心脏病患者108例作为研究对象,所有患者均行彩色多普勒超声心动图及数字减影血管造影检查,比较两组方法对先天性心脏病的检出情况,并以数字减影血管造影检查结果为金标准,分析彩色多普勒超声心动图诊断成人先天性心脏病的灵敏度、特异度、准确度、阳性预测值及阴性预测值;采用Kappa检验分析彩色多普勒超声心动图诊断先天性心脏病与数字减影血管造影的一致性。结果:108例疑似患者经数字减影血管造影检出78例先天性心脏病,其中彩色多普勒超声心动图检出75例,阳性检出率为96.15%(75/78);彩色多普勒超声心动图诊断成人先天性心脏病的灵敏度为93.59%(73/78),特异度为93.33%(28/30),准确度为93.52%(101/108),阳性预测值为97.33%(73/75),阴性预测值为84.85%(28/33);Kappa检验显示,TCCS检查诊断大脑中动脉狭窄与DSA检查一致性极好(Kappa值=0.843,P=0.000)。结论:彩色多普勒超声心动图检查成人先天性心脏病的效果理想,不仅具有较高的准确率,还具备较高的安全性,值得在临床上进一步推广与应用。Objective To analyze the application value of color Doppler echocardiography in diagnosing adult congenital heart disease.Methods A total of 108 patients with suspected adult congenital heart disease who were diagnosed and treated in Lanshan District People's Hospital of Rizhao City from August 2019 to August 2021 were selected as the research subjects.All patients underwent color Doppler echocardiography and digital subtraction angiography.The detection of congenital heart disease was analyzed by the two methods,and the results of digital subtraction angiography were used as the gold standard to analyze the sensi tivity,specificity,accuracy,positive predictive value and negative value of color Doppler echocardiography in the diagnosis of adult congenital heart disease.The Kappa test was used to analyze the agreement between color Doppler echocardiography and digital subtraction angiography in the diagnosis of congenital heart disease.Results 78 cases of congenital heart disease were detected by digital subtraction angiography in 108 suspected patients,of which 75 cases were detected by color Doppler echocardiography,with a positive detection rate of 96.15%(75/78).The sensitivity of electrocardiography in diagnosing adult congenital heart disease was 93.59%(73/78),the specificity was 93.33%(28/30),the accuracy was 93.52%(101/108),and the positive predictive value was 97.33%(73/75),the negative predictive value was 84.85%(28/33);the Kappa test showed that the color Doppler echocardiography examination in diagnosing adult congenital heart disease was in excellent agreement with the DSA examination(Kappa value=0.843,P=0.000).Conclusion Color Doppler echocardiography has ideal results in the examination of adult congenital heart disease.It not only has high accuracy,but also has high safety,and it is worthy of further promotion and application in clinical practice.
