周德安针刺治疗睑废的临床经验

摘　　要：眼肌型重症肌无力是一种常见的重症肌无力类型,主要局限于眼部周围肌肉受累,属中医“睑废”等范畴,严重影响患者生活质量。周德安教授善于在治疗临床病证中贯穿“治病先治神”理论,他认为睑废和五脏皆有关,其病位在眼,虚证病机为脾肾不足,实证为风痰阻络,肝郁化火,病因为气血瘀滞,胞络受阻,精气不能上承于胞睑,胞筋不养。针刺治疗应从治神、治痰、补益通调阳明经入手,辨证分虚实,虚则补之,实则泻之。Ocular myasthenia gravis is a common myasthenia gravis type,which is mainly limited to the involvement of muscles around the eyes,and it belongs to the category of ‘eyelid waste’ of TCM,which seriously affects the quality of patients’ life.Professor ZHOU De-an beleives that the theory of ‘acupuncture for the treatment of spirit’ is always the first go to all kinds of diseases.He thought that eyelid waste is related to the five-zang organs,it located in the eye and distinguished deficiency syndrome from excess syndrome,the pathogenesis of deficiency syndrome is the deficiency of spleen and kidney,while the one of excess syndrome are the obstruction of collateral caused by pathogenic wind and phlegm and the fire derived from stagnation of liver-qi.And the causes are qi stagnation and blood stasis,obstruction of cellular collaterals,vital essence cannot be transmitted to the eyelid,eyelid lack of nourishment.Acupuncture treatment should be started with the treatment of spirit,phlegm,regulating and tonifying yang ming meridian,be guided by syndrome differentiation of excess and deficiency,treating deficiency with tonification and excess with clearing.
