摘 要:目的探讨人补体受体1型双功能域短同源重复序列(CR1-SCR2D)对脓毒症小鼠炎症损伤的保护作用。方法清洁级雄性昆明小鼠随机分为假手术组(40只)、手术组(40只))和保护组(40只)。按参考文献方法构建盲肠结扎穿刺术(CLP)脓毒症小鼠模型,假手术组和手术组小鼠尾静脉注射无菌PBS 0.2 ml,保护组的小鼠注射CR1-SCR2D蛋白(10 mg/kg)。各组留10只小鼠观察手术后96 h生存率。其他小鼠在手术后24 h测定血清C5a、IL-6、TNF-α含量和肺组织MPO含量;免疫组化检测肺组织C4b和C3b的沉积;HE染色观察肺组织病理改变。结果脓毒症组的小鼠48 h后全部死亡,保护组的48 h死亡率为50%,96 h死亡率为80%,生存率显著提高(P<0.05)。脓毒症组的血清C5a、IL-6、TNF-α含量和肺组织MPO含量均明显升高,保护组的显著降低(P<0.05)。脓毒症组肺组织原位C4b和C3b沉积的平均光密度(MOD)值显著升高,保护组的MOD值显著低于脓毒症组的(P<0.05)。病理学检查显示,保护组小鼠肺损伤较脓毒症组的明显减轻。结论补体系统的过度活化在脓毒症的发生和发展中起重要作用,人CR1-SCR2D能明显减轻脓毒症小鼠炎症损伤,具有很好的保护作用。This study was preformed to investigate the protective effect of human complement receptor type 1 bifunctional short consensus repeat(CR1-SCR2D)on inflammatory injury in septic mice.Clean grade male Kunming mice were recruited and divided randomly into sham operation group(n=40),operation group(n=40)and protection group(n=40).The cecal ligation and puncture(CLP)sepsis mouse model was constructed according to the method of reference.Mice in the sham operation group and operation group were injected with 0.2 ml sterile PBS in the tail vein,and mice in the protection group were injected with CR1-SCR2D protein.The survival rate at 96 h after operation was observed in 10 mice in each group.The contents of C5a,IL-6 and TNF-αin serum and MPO in lung tissues of other mice were measured 24 h after operation.The deposition of C4b and C3b in lung tissue was detected by immunohistochemistry assay,and the pathological changes of lung tissues were observed by HE staining.Data showed that all mice in the sepsis group died at 48 h,while the mortality rate in the protected group was 50%at 48 h and 80%at 96 h.The contents of C5a,IL-6,TNF-αin serum,and MPO in lung tissue of the sepsis group were increased,while those in the protected group were significantly decreased(P<0.05),as compared with sham group.The average optical density(MOD)value of in situ C4b and C3b deposition in lung tissues of the sepsis group was significantly increased,and the MOD value of the protected group was significantly lower than that of the sepsis group(P<0.05).Pathological examination showed that lung damage of mice in the protective group was significantly alleviated compared to the sepsis group.The results suggest that the overactivation of the complement system plays an important role in the occurrence and development of sepsis,and human CR1-SCR2D can significantly reduce inflammatory injury in sepsis mice and has critical protective effects.
