摘  要:目的探讨巴马小型猪应用MRI定量成像及钆塞酸二钠增强扫描评估肝功能的可行性及应用价值。方法5只巴马小型猪均行肝脏MRI检查,平扫依次进行T_(2)WI、T_(1)WI、T_(1)mapping、T_(2)mapping、T_(2)^(*)mapping、多b值弥散加权成像和容积内插法体部检查(volume interpolated body examination,VIBE)序列扫描。增强扫描使用肝细胞特异性含钆对比剂钆塞酸二钠,血管期采用时间分辨交叉随机轨迹成像(time-resolved imaging with interleaved stochastic trajectories,TWIST)与VIBE联合序列(TWIST-VIBE),肝胆期于注射对比剂后20 min进行T_(1)mapping、VIBE序列扫描,30 min重复VIBE序列。图像经后处理获得灌注分数、纯扩散系数、伪扩散系数、平均扩散系数、平均峰度、平扫T_(2)值、平扫T_(2)^(*)值、平扫T_(1)值、肝胆期20 min时T_(1)值;测量肝胆期20、30 min时右叶肝实质及左侧竖脊肌VIBE序列信号值,获得肝脏/肌肉信号比值。结果5只小型猪右叶肝实质的灌注分数为0.32±0.08,纯扩散系数为(1.51±0.21)×10^(-3)mm^(2)/s,伪扩散系数为(50.38±22.92)×10^(-3)mm^(2)/s,平均扩散系数为(1.80±0.46)×10^(-3)mm^(2)/s,平均峰度为0.82±0.15,平扫T_(2)值为(33.06±4.18)ms,平扫T_(2)^(*)值为(6.81±0.83)ms,平扫T_(1)值为(704.47±29.41)ms,肝胆期20 min时T_(1)值为(428.80±25.06)ms。5只小型猪增强扫描血管期时间-信号强度曲线趋势基本一致,腹腔干血管期强化特征呈速升速降型,达峰时间为(17.80±4.38)s;门静脉主干呈速升缓降型,达峰时间为(69.00±9.80)s;下腔静脉呈速升速降型,达峰时间为(27.40±8.29)s;右叶肝实质呈缓慢上升趋势,达峰时间为(89.80±9.55)s。肝胆期20 min时T_(1)值[(428.80±25.06)ms]低于平扫T_(1)值[(704.47±29.41)ms](t=65.358,P<0.001);肝胆期30 min时肝脏/肌肉信号比值(2.03±0.17)与20 min时(1.98±0.15)比较差异无统计学意义(t=-2.203,P=0.092)。结论巴马小型猪行肝脏MRI定量成像及钆塞酸二钠增强扫描Objective To investigate the feasibility and value of MRI quantitative sequences and gadoxetic acid enhanced MRI to the evaluation of liver function of Bama minipigs.Methods Five Bama minipigs were selected for liver MRI scanning.The T_(2)WI,T_(1)WI,T_(1)mapping,T_(2)mapping,T_(2)^(*)mapping,multi-b-value diffusion imaging and volume interpolated body examination(VIBE)were performed in all minipigs respectively,and then gadoxetic acid was administrated for enhanced scan.Time-resolved imaging with interleaved stochastic trajectories(TWIST)-VIBE sequence was used for dynamic scanning in vascular phase;T_(1)mapping and VIBE sequence were performed in hepatobiliary phase 20 min after injection of contrast agent and VIBE sequence was repeated 30 min later.The perfusion fraction,pure diffusion coefficient,pseudo-diffusion coefficient,mean diffusion coefficient,mean kurtosis,unenhanced T_(2)value,unenhanced T_(2)^(*)value,unenhanced T_(1)value,and T_(1)in hepatobiliary phase were obtained by image processing.The signal values for VIBE sequence in the right liver lobe and left erector spinalis muscle were measured at 20and 30min in hepatobiliary phase,and the liver/muscle signal ratio was obtained.Results In the right liver lobe of these 5minipigs,the perfusion fraction was 0.32±0.08,the pure diffusion coefficient was(1.51±0.21)×10^(-3)mm^(2)/s,the pseudo-diffusion coefficient was(50.38±22.92)×10^(-3)mm^(2)/s,the mean diffusion coefficient was(1.80±0.46)×10^(-3)mm^(2)/s,the mean kurtosis value was 0.82±0.15,the unenhanced T_(2)value was(33.06±4.18)ms,the unenhanced T_(2)^(*)value was(6.81±0.83)ms,the unenhanced T_(1)value was(704.47±29.41)ms,and the T_(1)value in hepatobiliary phase was(428.80±25.06)ms.The time signal curves in vascular phase were basically consistent.The celiac trunk showed a rapid increase-rapid reduction pattern,and the peak time was(17.80±4.38)s;the portal vein showed a rapid increase-slow reduction pattern,and the peak time was(69.00±9.80)s;the inferior vena cava showed a rapid increas
