摘 要:目的分析广东省<15岁儿童急性弛缓性麻痹(Acute flaccid paralysis,AFP)病例肠道病毒病原学特征。方法对广东省2018-2021年AFP报告病例采集粪便标本,进行脊灰病毒(Poliovirus,PV)和非脊灰肠道病毒(Non-polio enterovirus,NPEV)分离和血清型鉴定,分析分离阳性率和血清型构成。结果广东省2018-2021年AFP病例肠道病毒分离总阳性率为5.26%(96/1824),其中PV、NPEV分别为0.27%(5例)、4.99%(91例);0岁、1-4岁、5-8岁、9-14岁病例NPEV分离阳性率分别为1.49%、9.17%、3.95%、0.97%,春季、夏季、秋季、冬季NPEV分离阳性率分别为6.57%、11.32%、2.22%、1.62%。共分离出9株PV,均为脊灰疫苗株,其中PV1、PV3、PV1+PV3血清型分别为1株、6株、2株。共分离出91株NPEV,其中肠道病毒A组6个血清型31株(34.07%)、B组15个血清型56株(61.54%)、C组2个血清型3株(3.30%)、待分型1株(1.10%);柯萨奇病毒(Coxsackie virus,CV)B3、CVA4、CVA2、CVB5血清型分别占20.88%、12.09%、8.79%、6.59%。结论广东省2018-2021年AFP病例监测未发现脊灰野病毒,继续维持无脊灰状态;NPEV感染以1-4岁儿童为主且呈夏季高峰,CVB3为NPEV优势流行血清型。Objective To analyze characteristics of pathogenic enteroviruses isolated from acute flaccid paralysis(AFP)cases<15 years of age in Guangdong province.Methods We collected stool specimens from AFP cases reported between 2018 and 2021 in Guangdong.We isolated polioviruses(PV)and non-polio enteroviruses(NPEV),and identified serotypes of the isolates to determine positivity rates and serotype distributions.Results The overall enterovirus positivity rate was 5.26%(96/1824)among the AFP cases;PV positivity was 0.27%(5 cases)and NPEV positivity was 4.99%(91 cases).NPEV positivity rates among 0-,1-4-,5-8-,and 9-14-year-olds were 1.49%,9.17%,3.95%,and 0.97%;by season,positive rates for spring,summer,autumn,and winter were 6.57%,11.32%,2.22%,and 1.62%.Nine PV strains were isolated,and all were polio vaccine strains:1 PV1,6 PV3,and 2 PV1+PV3.Ninety-one NPEV strains were isolated,including 31(34.07%)from 6 group A enterovirus serotypes,56(61.54%)from 15 group B serotypes,3(3.30%)from 2 group C serotypes,and 1(1.10%)unclassified strain.Coxsackie virus(CV)B3,CVA4,CVA2,and CVB5 accounted for 20.88%,12.09%,8.79%,and 6.59%of all NPEV strains.Conclusions No wild-type polioviruses were found in AFP cases in Guangdong from 2018 to 2021,indicating continuous polio-free status.The NPEV infection rate was high among 1-4-year-old children,and the rate peaked in the summer.CVB3 was the predominant NPEV serotype in the AFP cases.
