摘　　要：目的了解2020年天津市健康人群麻疹抗体水平及其影响因素。方法采用横断面调查方法在天津市4个区抽取0-44岁健康人群采集血清样本,采用酶联免疫吸附试验检测麻疹IgG抗体,分析抗体阳性率和几何平均浓度(Geometric mean concentration,GMC)以及抗体阳性率的影响因素。结果调查对象麻疹抗体总阳性率为87.93%(1282/1458),GMC为804.01mIU/mL。麻疹抗体阳性率从<8月龄的63.16%上升至2-4岁的96.30%,随后下降至15-19岁的81.03%,20-44岁各年龄组抗体阳性率在88.71%-91.11%之间。多因素Logistic回归分析显示,年龄、职业和含麻疹成分疫苗(Measles containing vaccine,MCV)接种后时间是麻疹抗体阳性率的显著影响因素;麻疹抗体阳性率随着接种后时间的推移而下降(OR=0.61,95%CI:0.52-0.72)。结论天津市2020年健康人群麻疹抗体水平总体偏低;需继续保持适龄儿童MCV高免疫覆盖率,探讨成人包括育龄期妇女MCV免疫策略。Objective To determine measles antibody levels and factors influencing measles seropositivity in a healthy population of Tianjin city in 2020.Methods We recruited 0-44-year-old healthy people from 4 districts of Tianjin for a cross-sectional measles serological survey.We obtained serum samples to detect measles IgG antibody using enzyme-linked immunosorbent assay.We determined seropositivity rates and geometric mean concentrations(GMCs)of antibody,and identified factors influencing seropositivity.Results The overall positivity rate was 87.93%(1282/1458)and the GMC of measles antibody was 804.01 mIU/mL.Positivity increased by age from 63.16%among<8-month-olds to 96.30%among 2-4-year-olds and then decreased to 81.03%among 15-19-year-olds.Positivity rates for the age groups among 20-44-year-olds ranged from 88.71%to 91.11%.Multivariate logistic regression showed that age,occupation,and time since vaccination with measles containing vaccine(MCV)significantly influenced measles antibody positivity.Positivity decreased with increasing time since vaccination(OR=0.61,95%CI:0.52-0.72).Conclusions Measles antibody levels were generally low among healthy people in Tianjin in 2020.We should maintain high MCV coverage among age-eligible children and explore MCV immunization strategies for adults,including non-pregnant women of childbearing-age.
