摘　　要：目的研究血清缺血修饰清蛋白(IMA)、微小RNA(miR)-214、可溶性ST2(sST2)在心力衰竭患者中的表达及预后价值。方法选取自2017年3月至2018年3月期间该院诊治的102例心力衰竭患者作为心衰组,按是否发生终点事件分为终点事件组(50例)和非终点事件组(52例)。以60例同期进行健康体检的体检健康者作为对照组。采用酶联免疫吸附试验检测各组血清sST2、IMA表达。采用荧光定量PCR检测各组血清miR-214的表达。统计学分析各组血清IMA、miR-214、sST2的表达水平差异。非条件多元Logistic回归模型分析影响心力衰竭预后的独立危险因素。采用受试者工作特征(ROC)曲线分析血清IMA、miR-214、sST2单独及联合检测对心力衰竭患者预后的预测价值。结果心衰组血清IMA、miR-214、sST2表达水平明显高于对照组,差异有统计学意义(P<0.05);重度心衰组血清IMA、miR-214、sST2表达水平明显高于轻度心衰组,差异有统计学意义(P<0.05)。终点事件组IMA、miR-214及sST2均高于非终点事件组,差异有统计学意义(P<0.05)。血清IMA、miR-214、sST2表达水平是心力衰竭预后的独立预测因素。血清IMA、miR-214、sST2及联合检测的曲线下面积(AUC)分别为0.728、0.705、0.798及0.931。与单项检测相比,联合检测血清IMA、miR-214、sST2对心力衰竭预后具有较高的预测价值(Z=3.727、4.220、3.378,P<0.001、0.001、0.001)。结论心力衰竭患者血清IMA、miR-214、sST2表达水平升高,三者是心力衰竭预后的独立危险因素。联合检测预测血清IMA、miR-214、sST2对心力衰竭患者预后预测的价值较高。Objective To study the expression level and prognostic value of serum ischemia modified albumin(IMA),microRNA(miR)-214,and soluble ST2(sST2)in patients with heart failure.Methods A total of 102 heart failure patients diagnosed and treated in the hospital from March 2017 to March 2018 were selected as the heart failure group.According to whether an endpoint event occurred,they were divided into the endpoint event group(50 cases)and the non-endpoint event group(52 cases).60 healthy people who had the physical examination during the same period were served as the control group.Enzyme-linked immunosorbent assay was used to detect the expression levels of serum sST2 and IMA in each group.Fluorescence quantitative PCR was used to detect the expression level of serum miR-214 in each group.The differences of serum IMA,miR-214,sST2 expression levels in each group were statistically analyzed.The unconditional Logistic binary regression model was used to analyze independent risk factors affecting the prognosis of heart failure.The receiver operating characteristic(ROC)curve was used to analyze the predictive value of single and combined detection of serum IMA,miR-214,sST2 on the prognosis of patients with heart failure.Results The expression levels of serum IMA,miR-214,and sST2 in the heart failure group was significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The expression levels of serum IMA,miR-214,and sST2 in the severe heart failure group was significantly higher than that in the mild heart failure group,the differences were statistically significant(P<0.05).The expression levels of serum IMA,miR-214,and sST2 in the end-point event group were higher than those in the non-end-point event group,and the differences were statistically significant(P<0.05).The expression levels of serum IMA,miR-214,and sST2 were independent predictors for the prognosis of heart failure.The area under the curve(AUC)of single and combined detection of serum IMA,miR-214,sST2 were 0.728
