摘 要:目的 构建并实施医养结合视角下初级养老护理员培训体系,对养老护理员、医护人员、老年患者进行满意度调查以评价其应用效果。方法 选取8个省市的15名函询专家经过两轮德尔菲专家函询确定各级指标;采用目的抽样法对300名养老护理员及其所属科室的200名医护人员、200名老年患者进行培训效果及满意度调查分析。结果 两轮函询问卷回收率为100%和93.75%,专家权威系数为0.818和0.837,重要性赋值均数在4.00~4.93,变异系数在0.07~0.23,肯德尔和谐系数(Kendall’s W)为0.158和0.275(P<0.01),由5个一级指标,19个二级指标和54个三级指标组成;经培训后,300名养老护理员理论知识得分为(80.25±7.43)分,实践技能得分为(81.73±6.81)分,均高于培训前,差异有统计学意义(P<0.01),参训率为100%,合格率为95%;与培训前比较,养老护理员及其所属科室医护人员、老年患者满意度均有不同程度提升(P<0.01)。结论 初级养老护理员培训体系指标设立科学可靠,该体系经实施后有效提升了养老护理员照护服务能力,提升了养老护理员、医护人员、老年患者的满意度,达到了理想的培训效果。Objective To construct and implement the training system of primary elderly care staff under perspective of combination of medical care, and to investigate satisfaction of elderly care staff, medical staff and elderly patients, and to evaluate application effect of the system. Methods Selected 15 correspondence experts from 8 provinces and cities to determine indicators with all levels through two rounds of Delphi expert correspondence;the purposeful sampling method was used to train 300 elderly care staff, 200 medical staff and 200 elderly patients in their departments and their satisfaction research analysis. Results The recovery rate of 2 round questionnaire was 100% and 93.75%, the expert authority coefficient was 0.818 and 0.837, the mean of importance assignment was between 4.00 and 4.93, and the coefficient of variation was between 0.07 and 0.23, and the Kendall harmony coefficient was 0.158 and 0.275(P<0.01). It consists of 5 first-level indicators, 19 second-level indicators and 54 third-level indicators;After training,the scores of theoretical knowledge(80.25 ± 7.43) and practical skills(81.73 ± 6.81) of 300 elderly care staff were higher than those before the training, and the difference was statistically significant(P<0.01). The training rate was100% and the pass rate was 95%;Compared with before training,the satisfaction of elderly care staff, medical staff in their departments and elderly patients were improved to varying degrees(P<0.01). Conclusion The index of the training system for primary elderly care staff is scientific and reliable. After implementation of the system, the nursing service ability of elderly care staff has been effectively improved, and the satisfaction of elderly care staff, medical staff and elderly patients has been improved, and the ideal training effect has been achieved.
