Tfh细胞亚群在肝移植术后HBV免疫应答中作用

摘　　要：目的探讨肝移植术后病人乙型肝炎(乙肝)疫苗注射对抗体产生的影响及其与滤泡辅助性T细胞(Tfh)细胞亚群的关系。方法选取2016—2019年在青岛大学附属医院行肝移植手术的病人为研究对象,采集9例符合纳入条件病人空腹肘静脉血,通过流式细胞术与ELISA法分别检测Tfh细胞亚群的细胞频率与乙肝表面抗体(HBsAb),通过CXC亚族趋化因子受体3和5(CXCR3、CXCR5)及CC趋化因子受体6(CCR6)的表达鉴别Tfh细胞亚群,并与受试者体内HBsAb进行相关性分析。结果肝移植术后乙肝疫苗接种者应答组为4例,未应答组为5例。应答组外周血Tfh和Tfh1细胞频率均显著高于非应答组(t=2.534、3.038,P<0.05),而外周血Tfh17细胞频率显著低于非应答组(t=3.872,P<0.05),两组外周血Tfh2细胞频率差异无显著性(t=0.853,P>0.05)。应答组体内HBsAb滴度与Tfh1/Tfh17比值呈正相关(r=0.982,P<0.05)。结论肝移植术后乙肝疫苗接种者体内HBsAb的产生可能依赖于外周血Tfh与Tfh1细胞的细胞频率,Tfh1与Tfh17细胞频率比值的偏移可能影响HBsAb的滴度。Objective To investigate the effect of hepatitis B vaccine injection on antibody production in patients after li-ver transplantation and its association with follicular helper T cell(Tfh)subsets.Methods The patients who underwent liver transplantation in The Affiliated Hospital of Qingdao University from 2016 to 2019 were enrolled as subjects,and cubital venous blood samples were collected in a fasting state from 9 patients who met inclusion criteria.Flow cytometry and ELISA were used to measure the frequency of Tfh subsets and HBsAb.Tfh subsets were identified by the expression of CXCR3,CXCR5,and CCR6,and their correlation with HBsAb was analyzed.Results Among the 9 patients,4 showed response to hepatitis B vaccine after liver transplantation(response group),while 5 had no such response(non-response group).Compared with the non-response group,the response group had significantly higher frequencies of peripheral blood Tfh and Tfh1 cells(t=2.534,3.038;P<0.05)and a significantly lower frequency of peripheral blood Tfh17 cells(t=3.872,P<0.05),while there was no significant difference in the frequency of Tfh2 cells in peripheral blood between the two groups(t=0.853,P>0.05).The titer of HBsAb was positively correlated with Tfh1/Tfh17 ratio in the response group(r=0.982,P<0.05).Conclusion The production of HBsAb in hepatitis B vaccine recipients after liver transplantation may depend on the frequency of Tfh and Tfh1 cells in peripheral blood,and the shift of Tfh1/Tfh17 ratio may affect the titer of HBsAb.
