轻重症腺病毒肺炎的肺功能改变以及临床分析

摘　　要：目的临床分析比较轻重症腺病毒肺炎的肺功能改变,为小儿腺病毒肺炎提供预后指导。方法随机选取2021年1月-2021年10月期间在佛山市妇幼保健院被诊断为轻症和重症腺病毒肺炎患者共60例,其中轻症30例,重症30例,另设正常对照组30例,观察并统计分析各组临床表现症状,肺功能相关测定参数情况,并统计分析并发症发生率。结果重症组在喘息和肺部湿啰音症状上,明显多于轻症组,差异均有统计学意义(P<0.05)。与正常对照组相比,重症组的呼气峰流速(PTEF)、达峰时间比(TPTEF/TE)和达峰容积比(VPTEF/VE)明显低于轻症组,差异均有统计学意义(P<0.05);轻症组的潮气量(VT/kg)、呼吸频率(RR)等均优于重症组,但差异均无统计学意义(P>0.05)。重症并发症发生率明显高于轻症,差异有统计学意义(P<0.05)。结论PTEF、TPTEF/TE和VPTEF/VE对于小儿腺病毒肺炎的预后更具有指导意义。Objective To clinically analyze the changes in lung function of mild and severe adenovirus pneumonia and provide prognostic guidance for children with adenovirus pneumonia Methods A total of 60 patients with mild and severe adenovirus pneumonia diagnosed in our hospital from January 2021 to October 2021 were randomly selected,including 30 mild cases,30 severe cases,and 30 normal control groups.Observation and statistical analysis of the clinical symptoms,pulmonary functicrrrelated measurement parameters of each group,and statistical analysis of the incidence of complications.Results The symptoms of wheezing and pulmonary rales in the severe group were significantly more than those in the mild group,and the differences were statistically significant(P<0.05).Compared with the normal control,the peak expiratory flow rate(PIEF),peak time ratio(TPIEF/1E)and peak volume ratio(VPIEF/VE)of the severe group were significantly lower than those of the mild group,and the differences were statistically significant(P<0.05);the tidal volume(VT/kg)and respiratory rate(RR)of the mild group were better than those of the severe group,but the difference was not statistically significant(P>0.05).The incidence of severe complications was significantly higher than that of mild cases,and the difference ms statistically significant(P<0.05).Conclusion PTEF,TPTEF/TE and VPTEF/VE have more guiding significance for the prognosis of children with adenovirus pneumonia.
