地佐辛预防无痛人流中麦角新碱不良反应的临床研究

摘　　要：目的观察地佐辛预防无痛人工流产(人流)中麦角新碱不良反应的有效性和安全性。方法选取2020年5-11月在该院经阴道彩色多普勒超声检查确诊为宫内早孕自愿行无痛人流术患者120例作为研究对象,采用随机数字表法分为对照组和观察组,每组60例。观察组患者术前5 min静脉推注地佐辛0.15 mg/kg,对照组患者静脉推注等量生理盐水,随后2组患者均给予丙泊酚2 mg/kg麻醉后行人流术,术毕均即刻臀部肌内注射麦角新碱0.2 mg。记录并比较2组患者入室时(T0)、麻醉前(T1)、麻醉后2 min(T2)、苏醒时(T3)生命体征(包括心率、平均动脉压、血氧饱和度等)、手术时间、呼吸抑制发生率、苏醒时间、丙泊酚用量、术中麻醉效果,以及术后2 h头痛、胸闷、面色潮红等不良反应发生情况等。结果2组患者手术时间、苏醒时间、各时间点生命体征比较,差异均无统计学意义(P>0.05)。观察组患者呼吸抑制发生率、丙泊酚用量,以及头痛、胸闷、面色潮红等不良反应总发生率均明显低于对照组,麻醉效果明显优于对照组,差异均有统计学意义(P<0.05)。结论地佐辛用于人流术具有良好的镇静镇痛效果,对呼吸的影响较小,血流动力学稳定,可安全、有效地预防麦角新碱引起的不良反应。Objective To observe the efficacy and safety of dezocine in preventing adverse reactions of ergometrine in painless induced abortion.Methods A total of 120 patients who were diagnosed with early intrauterine pregnancy by vaginal color Doppler ultrasound and voluntarily underwent painless abortion from May to November 2020 were selected as the research objects.They were divided into the control group and the observation group by random number table method with 60 cases in each group.Patients in the observation group were received intravenous injection of dezocine(0.15 mg/kg)five minutes before operation,while patients in the control group received intravenous injection of the same amount of normal saline.Subsequently,both groups were anesthetized with propofol 2 mg/kg before abortion,the patients in both groups were given ergometrine(0.2 mg)intramuscular injection immediately after the operation.The vital signs(including heart rate,mean arterial pressure,oxygen saturation,etc.),operation time,incidence of respiratory depression,recovery time,propofol dosage,intraoperative anesthetic effect,and the incidence of adverse reactions such as headache,chest tightness and flushing in two hours after operation were recorded and compared between the two groups at entry(T0),before anesthesia(T1),two minutes after anesthesia(T2),and after recovery(T3).Results There were no significant differences in operation time,recovery time and vital signs at each time point between the two groups(P>0.05).The incidence of respiratory depression,the dosage of propofol,the incidence of headache,chest tightness,flushing and other adverse reactions in the observation group were significantly lower than those in the control group,and the anesthetic effect in the observation group was significantly better than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Dezocine used in induced abortion has good sedative and analgesic effect,with little influence on respiration and stable hemodynamics,which ca
