COOK球囊与欣普贝生用于妊娠期高血压疾病引产的比较

摘　　要：目的比较妊娠期高血压疾病孕妇采用COOK球囊与欣普贝生引产过程中血压波动的情况。方法选取194例妊娠期高血压疾病患者,按照孕妇所选引产方式不同分为研究组(105例)与对照组(89例)。研究组使用COOK球囊,对照组使用欣普贝生。比较两组引产分娩过程中血压波动情况。结果在使用钙离子拮抗剂或者β受体阻滞剂降压治疗的所有孕妇中,研究组和对照组潜伏期、活跃期、第二产程、第三产程、产后血压的收缩压与舒张压最高值及催产后收缩压波动比较,差异无统计学意义(P>0.05)。但在未使用降压药物治疗孕妇中,两组潜伏期、活跃期、第二产程、第三产程的收缩压与舒张压最高值比较,差异具有统计学意义(P<0.05)。两组催产后收缩压波动比较,差异具有统计学意义(P<0.05)。结论COOK球囊与欣普贝生在妊娠期高血压疾病孕妇引产过程中对血压均有一定影响,但欣普贝生对血压影响更大,需在严密监护下使用。欣普贝生用于妊娠期高血压疾病引产时,需使用钙离子拮抗剂或β受体阻滞剂降压治疗的几率增加。Objective To compare the fluctuation of blood pressure in pregnant women with gestational hypertension during labor induction using COOK balloon and Simbube.Methods 194 patients with hypertensive disorders during pregnancy were selected and divided into study group(n=105)and control group(n=89)according to different methods of labor induction.COOK balloon was used in the study group,and Simpubesen was used in the control group.The fluctuation of blood pressure during labor induction was compared between the two groups.Results In all pregnant women treated with calcium antagonists or beta-blockers,there were no significant difFerences between the study group and the control group in the maximum values of systolic blood pressure and diastolic blood pressure during the incubation period,active period,second stage of labor,third stage of labor,postpartum blood pressure and post-oxytocin systolic blood pressure fluctuation(P>0.05).However,in pregnant women who were not treated with antihypertensive drugs,the maximum values of systolic blood pressure and diastolic blood pressure in the latent period,active period,second stage of labor,and third stage of labor were significantly different between the two groups(P<0.05).There was significant difference in systolic blood pressure fluctuation between the two groups(P<0.05).Conclusion Both COOK balloon and Simprobixen have certain effects on blood pressure during the induction of labor in pregnant women with gestational hypertension,but Simprobixen has a greater effect on blood pressure and should be used under strict supervision.When simprobixen is used for induction of labor in hypertensive disorders of pregnancy,the need for calcium antagonists or beta-blockers for antihypertensive therapy is increased.
