人工流产术后闭经和月经过少的原因及处理

摘　　要：目的探讨人工流产术后闭经和月经过少的原因及处理。方法选取40例闭经或者月经过少患者,患者均经宫腔镜检查、病理检查及相关激素水平检测,观察其检查结果。结果40例患者中宫腔内口黏连3例,宫腔黏连13例,宫腔瘢痕6例,宫内残留物4例,未见异常患者14例;患者均在宫腔镜检查后均行诊断性刮宫检查,40例患者均刮出内膜组织,其中增生期内膜25例,内分泌期内膜11例,内膜增生过长4例。在闭经患者中4例出现卵巢功能早退情况,3例高催乳血症,9例下丘脑功能失调;月经过少患者中排卵10例,无排卵14例。结论宫腔异常及卵巢功能失调是导致患者闭经或月经过少的主要原因,育龄期内妇女应采取积极有效的避孕措施,尽量降低或者避免人工流产情况,同时医务人员在实行流产手术中应严格按照操作常规进行,避免并发症及过度吸宫情况。Objective To explore the causes and treatment of amenorrhea and oligomenorrhea after induced abortion.Methods 40 patients with amenorrhea or minimal menstruation were selected.All the patients underwent hysteroscopy,pathological examination and related hormone level detection.Results Among the 40 patients,there were 3 cases of intrauterine mouth adhesion,13 cases of intrauterine adhesion,6 cases of intrauterine scar,4 cases of intrauterine residue,and 14 cases of no abnormality.All patients underwent diagnostic curettage examination after hysteroscopy.Intimal tissue was scraped out in all 40 patients,including 25 cases of membrane in hyperplasia period,11 cases of membrane in endocrine period,and 4 cases of hyperplasia.Among the amenorrhea patients,4 cases had premature ovarian function,3 cases had hyperprolactomia,and 9 cases had hypothalamic dysfunction.There were 10 cases of ovulation and 14 cases of anovulation.Conclusion Abnormal uterine cavity and ovarian dysfunction are the main causes of amenorrhea or oligomenorrhea.Women of childbearing age should take active and effective contraceptive measures to minimize or avoid the occurrence of induced abortion.At the same time,medical personnel should strictly follow the operation routine in the implementation of abortion to avoid complications and excessive uterine suction.
