不同孕激素治疗围绝经期无排卵型异常子宫出血的效果观察

摘　　要：目的观察不同孕激素治疗围绝经期无排卵型异常子宫出血的临床效果。方法选取236例围绝经期无排卵型异常子宫出血患者,按照随机信封分组法将患者分为对照组和研究组,各118例。对照组应用地屈孕酮单一药物进行治疗,研究组应用地屈孕酮联合复方烘诺酮进行治疗。对比两组患者止血情况、性激素水平、临床疗效及不良反应发生情况。结果研究组患者控制出血时间、完全止血时间均短于对照组,出血量少于对照组,差异具有统计学意义(P<0.05)。研究组患者卵泡刺激素、促黄体生成素、雌二醇水平均低于对照组,差异具有统计学意义(P<0.05)。研究组患者治疗总有效率98.31%高于对照组90.68%,差异具有统计学意义(P<0.05)。两组患者不良反应发生率对比,差异无统计学意义(P>0.05)。结论围绝经期无排卵型异常子宫出血患者应用地屈孕酮联合复方烘诺酮进行治疗能够缩短止血时间,降低性激素水平,提高临床疗效,值得临床推广与应用。Objective To observe the clinical effect of different progestins in the treatment of perimenopausal anovulatory abnormal uterine bleeding.Methods 236 perimenopausal patients with abnormal anovulatory uterine bleeding were selected and divided into control group and study group according to the random envelope grouping method,118 cases in each group.The control group was treated with dydrogesterone alone,and the study group was treated with dydrogesterone combined with compound norethinone.Hemostasis,sex hormone levels,clinical efficacy and adverse reactions were compared between the two groups.Results The bleeding control time and complete hemostasis time of the study group were shorter than those of the control group,and the amount of bleeding was less than that of the control group,and the differences were statistically significant(P<0.05).The levels of follicle stimulating hormone,luteinizing hormone and estradiol in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total effective rate of the study group was 98.31% higher than that of the control group 90.68%,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Dydrogesterone combined with compound norethistone in the treatment of perimenopausal patients with anovulatory abnormal uterine bleeding can shorten the hemostasis time,reduce the level of sex honnones,and improve the clinical efficacy,which is worthy of clinical promotion and application.
