浙江省德清县儿童其他感染性腹泻流行趋势及疾病负担分析

摘　　要：目的:分析2008-2020年浙江省德清县0-14岁儿童其他感染性腹泻(other-infectious diarrhea,OID)的流行趋势及疾病负担。方法:从中国疾病预防控制信息系统和德清县区域卫生信息平台获取数据,运用Joinpoint回归分析OID发病率变化趋势,采用伤残损失生命年(years lived with disability,YLDs)估算疾病负担,并估计直接医疗费用。结果:浙江省德清县儿童OID年均报告发病率为1237.63/10万,发病率呈小幅下降,男童较女童下降明显。0-2岁组儿童发病率下降明显,但是3-5岁组上升明显。11月至次年2月为发病高峰,病原学诊断以轮状病毒感染为主。儿童OID疾病负担YLDs为47.48人年、YLDs强度为5.85人年/10万,0-2岁组疾病负担最重。次均门诊费用中位数为157.43元,住院费用中位数为1932.13元。结论:德清县儿童OID发病率整体呈小幅下降趋势,但3-5岁年龄组儿童有上升趋势,疾病负担仍较高,针对OID重点人群应进一步采取健康教育、轮状病毒疫苗接种、病原学监测、幼托机构监管等综合防控措施。Objective:To analyze the epidemic trend and disease burden of other-infectious diarrhea(OID)in children aged 0-14 years old in Deqing county of Zhejiang province from 2008 to 2020.Methods:Data were obtained from China disease prevention and control information system and Deqing county regional health information platform.Joinpoint regression was used to analyze the trend of OID incidence,and years lived with disability(YLDs)and direct medical cost were estimated as the burden of disease.Results:The average annual reported incidence of OID in children was 1237.63/100000 in Deqing county of Zhejiang province,and there was a slight decrease in the incidence,with a marked decrease in boys compared with girls.The incidence statistically decreased among those aged 0 to 2 years old and it rose significantly among those aged 3 to 5 years old.The incidence reached at a peak between November to February of the following year.The etiological diagnosis was mainly rotavirus infection.The YLDs and YLDs intensity of OID among children were 47.48 person-years and 5.85 person-years/100000 respectively,with the highest burden of disease among those aged 0 to 2 years old.The median of average outpatient cost per visit was 157.43 yuan and the median of hospitalization cost was 1932.13 yuan.Conclusion:The overall incidence of OID decreased slightly among children in Deqing county but rose significantly among those aged 3 to 5 years old.Also,its disease burden was still heavy.Comprehensive prevention and control measures such as health education,rotavirus vaccination,etiological monitoring,and supervision of childcare institutions should be adopted to the key population of OID.
