摘　　要：目的探讨低剂量盐酸氨溴索联合双水平正压通气在不同胎龄早产儿呼吸衰竭中的临床效果。方法80例呼吸衰竭早产儿,根据应用盐酸氨溴索剂量分为A组与B组,各40例。A组为低剂量盐酸氨溴索配合双水平正压通气治疗,B组为高剂量盐酸氨溴索配合双水平正压通气治疗。A组与B组患儿再根据胎龄分为胎龄33~36周组和≤32周组。比较各组Silverman-Anderson(SA)评分、Downes评分、血气指标水平。结果A组中,33~36周组治疗0、12、24、36、48 h的SA评分分别为(7.39±0.45)、(5.32±0.41)、(4.18±0.36)、(3.41±0.32)、(2.39±0.30)分,Downes评分分别为(7.23±0.44)、(5.84±0.35)、(3.68±0.31)、(3.01±0.27)、(2.45±0.21)分;≤32周组治疗0、12、24、36、48 h的SA评分分别为(7.41±0.47)、(6.23±0.44)、(5.41±0.39)、(4.39±0.36)、(3.15±0.31)分,Downes评分分别为(7.25±0.46)、(6.34±0.45)、(5.12±0.41)、(4.51±0.36)、(3.52±0.24)分。B组中,33~36周组治疗0、12、24、36、48 h的SA评分分别为(7.38±0.44)、(5.87±0.43)、(5.01±0.40)、(4.64±0.38)、(3.41±0.36)分,Downes评分分别为(7.24±0.45)、(6.31±0.42)、(5.16±0.36)、(4.15±0.32)、(3.58±0.25)分;≤32周组治疗0、12、24、36、48 h的SA评分分别为(7.40±0.46)、(6.62±0.45)、(5.67±0.42)、(4.93±0.40)、(3.69±0.39)分,Downes评分分别为(7.26±0.48)、(7.01±0.32)、(5.98±0.45)、(5.01±0.39)、(4.21±0.30)分。治疗0 h,各组SA评分、Downes评分比较,差异无统计学意义(P>0.05);各组治疗12、24、36、48 h的SA评分、Downes评分低于本组0 h,差异有统计学意义(P<0.05);A组治疗12、24、36、48 h的SA评分、Downes评分低于B组,差异有统计学意义(P<0.05);A组及B组中33~36周组治疗12、24、36、48 h SA评分、Downes评分低于≤32周组,差异有统计学意义(P<0.05)。治疗0 h,各组pH、动脉-肺泡氧分压比值(a/A)、动脉血二氧化碳分压(PaCO_(2))、动脉血氧分压(PaO_(2))、气道阻力、肺顺应性比较,差异无统计学�Objective To discuss the clinical effect of low dose ambroxol hydrochloride combined with bi-level positive pressure ventilation on respiratory failure in preterm infants of different gestational ages.Methods A total of 80 premature infants with respiratory failure were divided into group A and group B according to the dosage of ambroxol hydrochloride,with 40 cases in each group.Group A was treated with lowdose ambroxol hydrochloride combined with bi-level positive pressure ventilation,and group B was treated with high-dose ambroxol hydrochloride combined with bi-level positive pressure ventilation.Children in group A and group B were divided into 33-36 weeks group and≤32 weeks group according to their gestational age.The Silverman-Anderson(SA)score,Downes score and blood gas indicators levels were compared in each group.Results In group A,the SA scores at 0,12,24,36,and 48 h after treatment in the 33-36 week group were(7.39±0.45),(5.32±0.41),(4.18±0.36),(3.41±0.32),and(2.39±0.30)points,and the Downes scores were(7.23±0.44),(5.84±0.35),(3.68±0.31),(3.01±0.27),and(2.45±0.21)points;the SA scores at 0,12,24,36,and 48 h of treatment in the≤32 week group were(7.41±0.47),(6.23±0.44),(5.41±0.39),(4.39±0.36),and(3.15±0.31)points,and the Downes scores were(7.25±0.46),(6.34±0.45),(5.12±0.41),(4.51±0.36),and(3.52±0.24)points.In group B,the SA scores at 0,12,24,36,and 48 h after treatment in the 33-36 week group were(7.38±0.44),(5.87±0.43),(5.01±0.40),(4.64±0.38),and(3.41±0.36)points,and the Downes scores were(7.24±0.45),(6.31±0.42),(5.16±0.36),(4.15±0.32),and(3.58±0.25)points;the SA scores at 0,12,24,36,and 48 h after treatment in the≤32 week group were(7.40±0.46),(6.62±0.45),(5.67±0.42),(4.93±0.40),and(3.69±0.39)points,and the Downes scores were(7.26±0.48),(7.01±0.32),(5.98±0.45),(5.01±0.39),and(4.21±0.30)points.At 0 h after treatment,there was no statistically significant difference in SA score and Downes score among the groups(P>0.05).The SA score and Downes score of each gro
