基于住院儿童患者地高辛有效浓度监测数据的回顾性分析

摘　　要：目的:回顾性分析住院儿童患者应用地高辛的情况,为临床使用该药提供更准确的有效浓度范围。方法:通过瑞美实验室管理系统,查询收集儿童患者病历,对数据进行统计和分析。结果:共收集病历150份,地高辛血药浓度为0.5~1.5 ng·mL^(-1)治愈率最高(69.5%),0.3 ng·mL^(-1)和0.3~0.5 ng·mL^(-1)浓度范围的治愈率(41.7%)、稳定率(33.3%)和已控制率(25.0%)均相同。男性血药浓度在0.3~0.5、1.5~2.0和>5.0 ng·mL^(-1)的比例高于女性,女性血药浓度在<0.3、0.5~1.5和2.0~5.0 ng·mL^(-1)的比例高于男性;1岁以下患者血药浓度范围在0.3~0.5 ng·mL^(-1)的比例低于1岁以上患者。结论:在实际临床治疗中,儿童患者的地高辛血药浓度低于目标浓度时,也能达到一定的治疗效果。为实现更合理用药,应结合儿童患者实际情况,个体化调整有效血药浓度范围。Objective:To retrospectively analyze the application of digoxin in hospitalized children,and provide a more accurate and effective therapy concentration range of digoxin in clinic.Methods:Medical records of the children were collected through the Ruimei laboratory management system and were statistically analyzed.Results:A total of 150 medical records were enrolled in this study.The cure rate of the children was the highest(69.5%)in a range of digoxin between 0.5 to 1.5 ng·mL^(-1).The cure rate(41.7%),stable rate(33.3%)and controlled rate(25.0%)of the children were the same in the range of digoxin as 0.3 ng·mL^(-1) and 0.3–0.5 ng·mL^(-1).Males at the range of digoxin as 0.3–0.5,1.5–2.0 and>5.0 ng·mL^(-1) were higher than females while females at the range of<0.3,0.5–1.5 and 2.0–5.0 ng·mL^(-1) were higher than males.In a range of digoxin between 0.3 to 0.5 ng·mL^(-1),the proportion of children under 1 year old was lower than that of children over 1 year old.Conclusion:The certain therapeutic effect of digoxin can also be achieved in children with lower concentration than the target,which indicated that individual effective concentration of digoxin should be selected based on their actual condition.
