摘　　要：目的探讨高血压相关性肾上腺肿瘤患者的临床特征。方法回顾性分析2010年1月至2020年1月该院收治的186例高血压相关性肾上腺肿瘤患者资料,将其分为功能性肿瘤组和无功能性肿瘤组,收集两组性别、年龄、BMI、肿瘤部位、肿瘤大小、肿瘤病理类型和合并症。比较两组临床特征,分析功能性肾上腺肿瘤相关危险因素。结果与无功能性肿瘤组比较,功能性肿瘤组年龄更小[(43.6±11.6)岁vs.(51.4±12.1)岁],肿瘤直径更长[(2.9±12.6)cm vs.(1.5±11.2)cm],合并有糖尿病(38.0%vs.20.4%)和脑血管疾病(23.4%vs.8.2%)的患者比例更高,差异有统计学意义(P<0.05)。多因素logistic回归分析结果显示,肿瘤直径≥2 cm(OR=5.038,95%CI:1.515~16.982)、合并糖尿病(OR=7.881,95%CI:2.517~25.369)、合并心血管疾病(OR=3.134,95%CI:1.109~9.459)和合并脑血管疾病(OR=6.264,95%CI:1.854~21.014)是功能性肿瘤的独立危险因素(P<0.05)。结论应针对高血压相关功能性肾上腺肿瘤危险因素进行干预。Objective To explore the clinical characteristics of the patients with hypertension-related adrenal tumors.Methods The data of 186 patients with hypertension-related adrenal tumors treated in this hospital from January 2010 to January 2020 were retrospectively analyzed,dividing the patients into the functional adrenal tumor group and the non-functional adrenal tumor group.The age,gender,BMI,tumor location,tumor size,tumor type and associated diseases of the two groups were collected.The clinical characteristics of the two groups were compared and the risk factors of functional adrenal tumors were analyzed.Results Compared with those of the non-functional tumor group,the patients in the functional tumor group were younger[(51.4±12.1)years old vs.(43.6±11.6)years old],longer tumor diameter[(1.5±11.2)cm vs.(2.9±12.6)cm],and more people complicated with diabetes(20.4%vs.38.0%)and cerebrovascular diseases(8.2%vs.23.4%).The differences were statistically significant(P<0.05).The results of the multivariate logistic regression analysis showed that the tumor diameter≥2 cm(OR=5.038,95%CI:1.515-16.982),combined with diabetes mellitus(OR=7.881,95%CI:2.517-25.369),combined with cardiovascular disease(OR=3.134,95%CI:1.109-9.459)and combined with cerebrovascular disease(OR=6.264,95%CI:1.854-21.014)were the independent risk factors for functional tumors(P<0.05).Conclusion The intervention should be carried out on the risk factors of the hypertension-related functional adrenal tumors.
