摘  要:目的探讨哌甲酯联合认知行为功能训练对注意缺陷多动障碍(ADHD)患儿的疗效。方法收集漯河市第二人民医院收治的86例ADHD患儿,按治疗方案分成A组、B组,各43例。B组接受哌甲酯治疗,A组接受哌甲酯联合认知行为功能训练治疗。对比两组总有效率、治疗前后ADHD核心症状评估量表评分、执行功能、韦氏儿童智力量表(C-WISC)评分、血清脑源性神经营养因子(BDNF)、胶质源性神经营养因子(GDNF)水平。结果A组总有效率较B组高(P<0.05)。治疗3个月后,A组注意缺陷、对立违抗、多动冲动评分较B组低,反应时间较B组短,漏报数、错认数较B组少,数字广度、算数、编码评分较B组高,血清GDNF、BDNF水平较B组高(P<0.05)。结论哌甲酯联合认知行为功能训练治疗ADHD患儿效果较好,可有效调节血清GDNF、BDNF水平,提高执行功能及认知功能,改善核心症状。Objective To explore the effect of methylphenidate combined with cognitive behavioral function training in children with attention deficit hyperactivity disorder(ADHD).Methods A total of 86 children with ADHD in Luohe Second People’s Hospital were collected and divided into group A and group B according to treatment plans,with 43 cases in each group.Group B received methylphenidate treatment,and group A received methylphenidate combined with cognitive behavioral function training.The total effective rate,ADHD core symptom assessment scale score,executive function,Wechsler children’s intelligence scale(C-WISC)score,serum brain-derived neurotrophic factor(BDNF),and glial-derived neurotrophic factor(GDNF)were compared between the two groups before and after treatment.Results The total effective rate of group A was higher than that of group B(P<0.05).After 3 months of treatment,the scores of attention deficit,oppositional defiant and hyperactivity of group A were lower than those of group B,the reaction time was shorter than that of group B,and the number of missed reports and misidentifications were less than those of group B,and the scores of number span,arithmetic and coding were higher than those of group B,and the levels of serum GDNF and BDNF were higher than those of group B(P<0.05).Conclusion Methylphenidate combined with cognitive behavioral function training has exact effect in the treatment of ADHD children,which can effectively regulate serum GDNF and BDNF levels,improve executive function and cognitive function,and improve core symptoms.
