酒石酸美托洛尔缓释片联合培哚普利对慢性扩张型心肌病心功能不全患者的疗效

摘　　要：目的探讨酒石酸美托洛尔缓释片联合培哚普利治疗慢性扩张型心肌病(DCM)心功能不全患者的效果。方法选取2018年1月至2019年12月内乡县人民医院收治的146例慢性DCM心功能不全患者,按治疗方案分为单药治疗组(n=73)和联合药物治疗组(n=73)。单药治疗组接受培哚普利治疗,联合药物治疗组在单药治疗组基础上加用酒石酸美托洛尔缓释片。比较两组总有效率、治疗前后的心功能、运动耐力、IL-33/ST2信号通路表达、不良反应发生率及治疗后6个月主要不良心血管事件(MACE)发生率。结果联合药物治疗组总有效率高于单药治疗组(P<0.05);治疗后,联合药物治疗组左室射血分数高于单药治疗组,6分钟步行距离(6MWD)长于单药治疗组,左室舒张末期内径、左室收缩末期内径、心率低于单药治疗组(P<0.05);治疗后,联合药物治疗组血浆白细胞介素-33、生长刺激表达基因2蛋白水平低于单药治疗组(P<0.05);联合药物治疗组MACE发生率低于单药治疗组(P<0.05)。结论酒石酸美托洛尔缓释片联合培哚普利可改善慢性DCM心功能不全患者的心功能及运动耐力,同时调控IL-33/ST2信号通路表达,可降低MACE发生风险,治疗安全性良好。Objective To investigate the clinical effects of metoprolol tartrate sustained-release tablets combined with perindopril in patients with chronic dilated cardiomyopathy(DCM)with cardiac insufficiency.Methods A total of 146 patients with chronic DCM cardiac insufficiency in Neixiang County People’s Hospital from January 2018 to December 2019 were selected and divided into single drug treatment group(n=73)and combined drug treatment group(n=73)according to treatment schemes.The single drug treatment group were treated with perindopril,and the combined drug treatment group were treated with metoprolol tartrate sustained-release tablets on the basis of the single drug treatment group.The total effective rate,cardiac function,exercise tolerance,IL-33/ST2 signaling pathway expression before and after treatment,incidence of adverse reactions and incidence of major adverse cardiovascular events(MACE)after 6 months of treatment were compared between the two groups.Results The total effective rate of combined drug treatment group was higher than that of single drug treatment group(P<0.05).After treatment,the left ventricular ejection fraction in combined drug treatment group was higher than that in single drug treatment group,the 6-minute walking distance(6MWD)was longer than that in single drug treatment group,and the left ventricular end diastolic diameter,left ventricular end systolic diameter and heart rate were lower than those in single drug treatment group(P<0.05).The plasma levels of interleukin-33 and growth-stimulating expression gene 2 protein in combined drug treatment group were lower than those in single drug treatment group after treatment(P<0.05).The incidence of MACE in combined drug treatment group was lower than that in single drug treatment group(P<0.05).Conclusion Metoprolol tartrate sustained-release tablets combined with perindopril can improve the cardiac function and exercise tolerance of patients with chronic DCM with cardiac insufficiency,and regulate the expression of IL-33/ST2 signaling pathw
