三种子宫内膜炎家兔模型的建立与评价

摘  要:为建立一种理想的子宫内膜炎模型,试验选取24只日本大耳雌兔,随机分成4组,即对照组、乙醇组、菌液组和苯酚组,每组6只,通过腹腔手术方式对各组左侧子宫分别注射生理盐水、95%乙醇、大肠杆菌和金黄色葡萄球菌混合菌液及25%苯酚凝胶各1 mL,通过临床症状观察、B型超声波检查、血常规炎性相关指标(白细胞、颗粒细胞、淋巴细胞/单核细胞)检测、组织病理学检查和细胞因子检测系统评价四种诱导试剂对家兔子宫内膜炎的诱导效果。结果表明:对照组精神、食欲未见异常,其他各组均出现精神沉郁,食欲不振。与对照组比较,其他三组子宫明显可见,且子宫体变宽,子宫黏膜增厚;乙醇组、菌液组和苯酚组的白细胞、颗粒细胞数量均极显著增加(P<0.01);乙醇组的淋巴细胞/单核细胞显著增加(P<0.05),苯酚组和菌液组的淋巴/单核细胞极显著增加(P<0.01);其中乙醇组引起的白细胞和颗粒细胞变化较为明显,菌液组引起的淋巴/单核细胞变化较为明显。乙醇组左侧子宫出现明显水肿、出血、坏死;苯酚组子宫左侧子宫肿大,但不见出血和坏死;菌液组双侧子宫均出现水肿,未见出血及坏死,其水肿程度小于苯酚组和乙醇组。乙醇组组织纤维蛋白渗出、水肿,含铁血红素出现,巨噬细胞浸润;菌液组固有层浅层淋巴细胞浸润,腺管萎缩,纤维蛋白渗出,组织水肿;苯酚组组织纤维蛋白渗出,肌纤维条呈索状,巨噬细胞浸润,组织水肿。与对照组比较,苯酚组、菌液组和乙醇组的白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)含量均极显著升高(P<0.01),其中乙醇组各炎症细胞因子含量均为最高。说明三种诱导试剂均可成功诱导家兔发生子宫内膜炎,但比较而言,乙醇诱导的效果最好。In order to establish an ideal model of endometritis,twenty-four Japanese female rabbits with large ears were randomly divided into four groups,namely control group,ethanol group,bacteria solution group and phenol group,with six rabbits in each group.The left uterus of each group was injected with 1 mL of normal saline,95%ethanol,mixed bacteria solution of Escherichia coli and Staphylococcus aureus and 25%phenol gel respectively by abdominal surgery.The effect of four induction reagents on rabbit endometritis was evaluated by clinical symptom observation,B-type ultrasonic examination,blood routine inflammatory indicators(white blood cell,granulosa cell,lymphocyte/monocyte)detection,histopathological examination and cytokine detection system.The results showed that there were no abnormalities in spirit and appetite in the control group,but depression and inappetence in other groups.Compared with the control group,the uterus in the other three groups could be seen,and the uterus body was wider and the uterine mucosa was thickened.The number of WBC and granulosa cells in ethanol group,bacteria liquid group and phenol group were significantly increased(P<0.01).The lymphocyte/monocyte ratio in ethanol group was significantly increased(P<0.05),and the lymphocyte/monocyte ratio in phenol and bacteria liquid groups was extremely significantly increased(P<0.01).The changes of white blood cells and granulosa cells were more obvious in ethanol group,and the changes of lymphocytes/monocytes were more obvious in bacteria liquid group.In ethanol group,the left uterus showed obvious edema,hemorrhage and necrosis.In the phenol group,the uterus on the left side of the uterus was enlarged,but there was no bleeding and necrosis.There was edema in both sides of the uterus in the bacteria solution group,but neither bleeding nor necrosis was observed,and the degree of edema was less than that in the phenol and ethanol groups.In ethanol group,fibrin exudate and edema,methemohaemoglobin appeared and macrophages infiltrated.In the bacter
