摘  要:目的:探讨核磁共振技术中半傅里叶单次激励快速自旋回波(HASTE)和真实稳态进动快速成像(true-FISP)影像组学模型联合血游离β绒毛膜促性腺激素(β-HCG)产前预测胎盘植入性病变价值。方法:回顾性收集本院2018年2月-2021年3月收治的137例疑似胎盘植入性病变患者临床资料,以手术病理结果作为金标准分为阳性组(n=123)和阴性组(n=14),均接受HASTE和true-FISP影像组学模型检查,检测血清β-HCG,分析HASTE、true-FISP检查阳性率和血清β-HCG水平,采用受试者工作特征曲线(ROC)分析HASTE、true-FISP影像组学模型及血清β-HCG产前预测胎盘植入性病变价值。结果:137例疑似胎盘植入患者,经术后病理确诊126例,诊断阳性率HASTE检查为88.6%;true-FISP检查为85.4%;阳性组血清β-HCG水平高于阴性组(P<0.05);产前胎盘植入性病变敏感度、特异度,HASTE分别为88.6%、42.9%、true-FISP分别为85.4%、50.0%,β-HCG分别为86.2%、64.3%,3项指标联合的敏感度(96.7%)、特异度(85.7%)最高(P<0.05)。结论:HASTE、true-FISP影像组学模型联合血β-HCG产前诊断胎盘植入性病变价值较高,值得临床参考应用。Objective:To investigate the value of radiomics-based model of half-Fourier acquisition single-shot turbo spin-echo(HASTE)and true fast imaging with steady-state precession(true-FISP)combined with free beta-human chorionic gonadotropin(β-HCG)level for predicting placenta accreta.Methods:The clinical data of 137 women with suspected placenta accreta from February 2018 to March 2021 were collected retrospectively,and these women had received examination of radiomics-based model of HASTE and true-FISP.These women were divided into group A(123 women with positive results)and group B(14 women with negative results)according to the pathological results after surgery as the gold standard.The women in the two groups were given serumβ-HCG level detection.The positive rate of HASTE and true-FISP,and the serumβ-HCG level of the women in the two groups were analyzed.Receiver operating characteristic(ROC)curve was used to evaluate the predictive value of radiomics-based model of HASTE and true-FISP and theβ-HCG level for placenta accreta.Results:Among 137 women with suspected placenta accreta,126 cases were confirmed by postoperative pathology,and the positive rates of placenta accreta by HASTE and true-FISP were 88.6%and 85.4%.The level of serumβ-hCG of the women in group A was significantly higher than that of the women in group B(P<0.05).The sensitivity and the specificity of HASTE for predicting placenta accreta were 88.6%and 42.9%,the sensitivity and the specificity of true-FISP for predicting placenta accreta were 85.4%and 50.0%,and the sensitivity and the specificity of theβ-HCG level for predicting placenta accreta were 86.2%and 64.3%.The sensitivity and the specificity of HASTE and true-FISP combined with heβ-HCG level for predicting placenta accreta were 96.7%and 85.7%(P<0.05).Conclusion:Radiomics-based model of HASTE and true-FISP combined with theβ-HCG level for diagnosis of the antenatal placenta accreta of the pregnant women is higher,which is worthy of application in clinic.
