摘 要:目的:比较刮片法和皮肤镜法在疥疮诊断中的应用价值。方法:收集2020年5月一2021年2月在成都市第二人民医院皮肤科门诊110例拟诊为疥疮的患者,每例同时进行刮片法和皮肤镜的检查,对检查结果进行配对四格表卡方检验;研究两种方法结果是否具有一致性,以Kappa检验评价一致性程度。结果:110例患者中,皮肤镜阳性59例,刮片法阳性38例,皮肤镜阳性率为54%,明显高于刮片法的35%,差异有统计学意义(χ^(2)=14.81,P<0.05);刮片法和皮肤镜检查结果一致性程度较好,(Kappa=0.52,P<0.01)。结论:刮片法与皮肤镜法对疥疮的检查结果有较好的一致性,两种方法联合使用可为疥疮的临床诊断治疗提供更好的实验室依据。Objective: To compare light microscopy examination of a skin scraping and dermatoscopy as diagnostic methods for scabies. Methods: From May 2020 to February 2021, 110 patients with suspected scabies seen in department of dermatology of Chengdu Second People’s hospital were selected. Each of these 110 patients was examined by light microscopy of a skin scraping(scraping method) and dermatoscopy, and the results were compared with a four-panel table card test. The consistency of the results of these two methods was evaluated using Kappa test. Results: Among these 110 patients, 59 cases were positive by dermoscopy, and 38 cases were positive by scraping method. The positive rate of dermatoscopy was 54%, which was significantly higher than that of the scraping method(35%). The difference was statistically significant( χ^(2)=14.81,P<0.05).The results of scraping method and dermatoscopy are comparable(Kappa=0.52,P<0.01). Conclusion: Light microscopic examination of skin craping and dermatoscopy show comparable results as diagnostic methods for scabies, and using both methods can provide a better laboratory basis for the clinical diagnosis and treatment of scabies.
