早期肠内联合肠外营养对神经重症患者短期疗效的评价

摘　　要：目的:分析神经外科重症患者早期单独肠内营养与肠内联合肠外营养的短期疗效比较。方法:回顾性分析某医院神经外科重症监护室(NICU)收治的277例重度颅脑损伤患者的资料,根据营养方式分为肠内营养组和联合营养组,用倾向评分匹配方法(PSM)处理,两组各纳入97例,比较两组患者治疗后的血液学营养指标和并发症。结果:PSM后两组患者临床无统计学差异,治疗后联合肠内营养组患者的血清白蛋白、血清总蛋白、血清前白蛋白和淋巴细胞计数比单独营养组高,在治疗期间腹胀、胃潴留和肺部感染的发生率较单独营养组低,差异均有统计学意义(P<0.05)。结论:肠内营养联合肠外营养较单独肠内营养能够改善神经重症患者的短期营养状态和降低治疗期间的并发症发生率。Objective:This paper analyzes the short-term efficacy of early enteral nutrition alone and enteral combined parenteral nutrition in severe neurosurgical patients.Methods:277 patients with severe craniocerebral injury in Neurosurgical intensive Care Unit(NICU)of the Hospital were retrospectively analyzed.They were divided into enteral nutrition group and combined nutrition group according to postoperative nutrition.The tendency score matching method(PSM)was used and 97 patients were put in each group.The hematological nutritional indexes and complications of the two groups were compared.Results:there was no statistical difference between the two groups after PSM.After treatment,the serum albumin,serum total protein,serum prealbumin and lymphocyte count in the combined enteral nutrition group were higher than those in the single nutrition group.During the treatment,the incidence of abdominal distension,gastric retention and pulmonary infection in the combined enteral nutrition group was significantly lower than that in the single nutrition group.Conclusions:enteral nutrition combined with parenteral nutrition can improve the short-term nutritional status of neurological severe patients and reduce the incidence of complications during treatment.
