摘 要:目的探讨胸部CT与X线胸片放射诊断与鉴别诊断良性肿瘤和肺癌的临床价值。方法选取2019年12月—2021年8月邳州市人民医院收治的60例良性肿瘤与肺癌患者作为本次研究对象,所有患者均进行X线胸片放射诊断、胸部CT诊断两种方式,分析两种诊断方式下的影像学表现、疾病诊断准确率、特异性与敏感度,并以病理结果作为诊断的金标准。结果两种诊断方式在锯齿状或毛刺、边缘不规则或分叶征、结节症、全肺不张或肺叶一侧、其他方面差异有统计学意义(P<0.05)。病理分析结果:良性肿瘤36例,肺癌24例;胸部CT诊断结果显示:良性肿瘤确诊率为94.44%,肺癌确诊率为95.83%;X线胸片放射诊断显示:良性肿瘤确诊率为36.11%,肺癌确诊率为50.00%,差异有统计学意义(χ^(2)=27.023、12.765,P<0.05)。胸部CT诊断准确率、特异性、敏感度高于X线胸片放射诊断结果,差异有统计学意义(χ^(2)=39.435、12.765、27.023,P<0.05)。结论CT诊断会避免病变与正常肺组织重叠显示图像,且密度分辨率较高,CT诊断可为患者提供准确结论,值得应用。Objective To investigate the clinical value of chest CT and X-ray in the radiological diagnosis and differential diagnosis of benign tumors and lung cancers.Methods Selected 60 benign tumors and lung cancer patients received by the People′s Hospital of Lizhou City from December 2019 to August 2021 as the subject of the study.All patients were diagnosed by X-ray chest radiography and chest CT.The imaging manifestations,disease diagnosis accuracy,specificity and sensitivity of the two diagnostic methods were analyzed,and the pathological results were used as the gold standard for diagnosis.Results There were statistically significant differences in serration or burr,irregular edge or lobular sign,sarcoidosis,whole lung atelectasis or one side of lung lobe,and other aspects between the two diagnosis method(P<0.05).Pathological analysis results:36 cases of benign tumors and 24 cases of lung cancer;the results of chest CT diagnosis showed that the diagnosis rate of benign tumors was 94.44%,and the diagnosis rate of lung cancer was 95.83%;radiographic diagnosis of chest X-ray showed that the diagnosis rate of benign tumors was 36.11%,and the diagnosis rate of lung cancer was 50.00%,the difference was statistically significant(χ^(2)=27.023,12.765,P<0.05).The diagnostic accuracy,specificity and sensitivity of chest CT were higher than that of X-ray chest radiography,and the difference was statistically significant(χ^(2)=39.435,12.765,27.023,P<0.05).Conclusion CT diagnosis can avoid overlapping images of lesions and normal lung tissue,and has high density resolution.CT diagnosis can provide accurate conclusions for patients,which is worthy of application.
