摘  要:目的研究胎先露在不同公式中对胎儿体质量超声估计的影响,并予以分析,以期为临床超声估计胎儿体质量(estimated fetal weight,EFW)提供指导依据。方法选取2017年1月—2022年1月于广州中医药大学附属宝安中医院分娩的胎儿220例。将其按照胎先露的差异分为研究组(臀先露)110例及对照组(头先露)110例。在最后1次超声检查至分娩时间间隔3 d内进行超声估计胎儿体质量(sonographic estimated fetal weight,sEFW)。采用21个已发表的sEFW公式估计EFW,并按照公式中所含有的胎儿生物测量指数组合,细分为六组(第1组包括AC与FL,第2组包括AC及BPD,第3组包括AC及HC(±BPD),第4组包括AC、FL及BPD,第5组包括AC、FL及HC;第6组包括AC、FL、BPD及HC)。比较两组BPD、HC、AC及FL,通过不同公式计算EFW对胎儿出生体质量预测的符合率,用不同公式计算EFW和出生体质量的克隆巴赫α值。结果研究组HC以及FL分别为(338.46±13.42)、(72.00±2.49)mm,均高于对照组,差异有统计学意义(t=3.749、2.012,P<0.05)。研究组细分6组公式计算EFW对胎儿出生体质量预测的符合率均低于对照组,差异有统计学意义(P<0.05)。研究组细分6组公式计算EFW和出生体质量的克隆巴赫α值均低于对照组,差异有统计学意义(P<0.05)。结论胎先露对不同公式计算EFW具有一定的影响,其中臀先露对sEFW的影响程度相较于头先露更大。Objective To study the influence of fetal presentation on the ultrasound estimation of fetal weight in different formulas and analyze it,in order to provide guidance for clinical ultrasound estimation of fetal weight(EFW).Methods 220 fetuses delivered in Bao'an Hospital of Traditional Chinese Medicine Affiliated to Guangzhou University of Traditional Chinese Medicine from January 2017 to January 2022 were se⁃lected.They were divided into 110 cases in the study group(breech presentation)and 110 cases in the control group(head presentation)ac⁃cording to the difference of fetal presentation.Fetal body mass(sEFW)was estimated by ultrasound within 3 days between the last ultrasound examination and delivery.Using 21 published sefw formulas to estimate EFW,and according to the combination of fetal biometric indexes contained in the formula,it was divided into six groups(group 1 included AC and FL,group 2 included AC and BPD,group 3 included AC and HC(±BPD),group 4 included AC,FL and BPD,and group 5 included AC,FL and HC;group 6 included AC,FL,BPD,and HC).Compared the two groups of BPD,HC,AC and FL,calculated the coincidence rate of EFW on the prediction of fetal birth weight through different formulas,and calculated the clonbach of EFW and birth weight with different formulasαValue.Results HC and FL in the study group were(338.46±13.42)mm and(72.00±2.49)mm,respectively,which were higher than those in the control group,and the difference was statistically significant(t=3.749,2.012,P<0.05).The study group was subdivided into 6 groups of formulas to calculate the coincidence rate of EFW for the prediction of fetal birth weight,all of which were lower than those of the control group,and the difference was statistically significant(P<0.05).The Cronbach'sαvalues of EFW and birth weight calculated by the formula in the study group were subdivided into 6 groups,which were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion Fetal presentation has a cer⁃tain influen
