中医药治疗糖尿病周围神经病变的用药及Meta分析

摘　　要：目的探讨中医药治疗糖尿病周围神经病变(DPN)的用药有效性及安全性。方法以中国知识资源总库(CNKI)为检索库,检索治疗糖尿病周围神经病变的中药方剂,并通过计算机检索PubMed、Web of science、Cochrane Library、Embase和中文检索国学术期刊数据库(万方数据)、中国生物医学文献数据库(CBM)、中国知识资源总库(CNKI)、中文科技期刊数据库(维普网),筛选2021年3月之前报道补阳还五汤治疗糖尿病周围神经病变的随机对照试验;文献入选标准:对照组采用基础治疗合西医治疗,治疗组采用补阳还五汤合对照组治疗;采用临床有效率、正中神经运动及感觉神经传导速度、腓总神经运动及感觉神经传导速度作为结局指标。对文献进行质量评价,纳入合格文献,采用RevMan 5.3软件进行Meta分析。结果统计出98首治疗DPN的方剂,前三位其次为补阳还五汤(21次),黄芪桂枝五物汤(18次),益气活血通痹汤(8次);纳入的9个RCT研究,提示补阳还五汤联合西药常规治疗DPN在疗效、神经传导、血液流变学等方面均优于单纯使用西药常规治疗,差异性具有统计学意义。结论补阳还五汤可安全有效的用于治疗糖尿病周围神经病变。Objective To investigate the efficacy and safety of traditional Chinese medicine in the treatment of diabetic peripheral neuropathy.Methods Chinese medicine prescriptions for diabetic peripheral neuropathy were retrieved from CNKI.PubMed,web of science,Cochrane Library,EMBASE,Wanfang database,CBM and CNKI were searched by computer to screen the randomized controlled trials of Buyang Huanwu Decoction(补阳还五汤)in the treatment of diabetic peripheral neuropathy before March 2021.Literature selection criteria:The control group was treated with basic treatment combined with western medicine,and the treatment group was treated with Buyang Huanwu Decoction combined with the basic treatment.The clinical effective rate,median nerve motor and sensory nerve conduction velocity,common peroneal nerve motor and sensory nerve conduction velocity were used as the outcome indicators.The quality of the literature was evaluated and the qualified RCTs were included.Revman 5.3 software was used for Meta-analysis.Results A total of 98 prescriptions for the treatment of diabetic peripheral neuropathy were counted.The top three were Buyang Huanwu Decoction(21 times),Huangqi Guizhi Wuwu Decoction(黄芪桂枝五物汤)(18 times),Yiqi Huoxue Tongbi Decoction(益气活血通痹汤)(8 times).Nine included RCT studies suggested that Buyang Huanwu Decoction combined with conventional western medicine in the treatment of diabetic peripheral neuropathy was superior to conventional western medicine in terms of efficacy,nerve conduction,hemorheology and the difference was statistically significant.Conclusion Buyang Huanwu Decoction is safe and effective in the treatment of diabetes peripheral neuropathy.
