鲜益母草胶囊联合屈螺酮炔雌醇片治疗人工流产术后出血临床研究

摘　　要：目的:观察鲜益母草胶囊联合屈螺酮炔雌醇片治疗人工流产术后出血的临床疗效。方法:选取86例人工流产术后出血患者,按随机数字表法分为观察组与对照组各43例。对照组采用屈螺酮炔雌醇片治疗,观察组采用鲜益母草胶囊联合屈螺酮炔雌醇片治疗。比较2组临床疗效、月经恢复时间、阴道出血量及阴道出血时间;比较2组治疗前后子宫内膜厚度、血流动力学相关指标及炎症因子水平。结果:治疗后,观察组总有效率为90.70%,高于对照组69.77%(P<0.05)。观察组阴道出血时间、阴道出血量较对照组减少(P<0.05),月经恢复时间早于对照组(P<0.05)。治疗后,2组子宫内膜厚度、子宫内膜螺旋动脉血流量(PBF)均较治疗前增加(P<0.05),波动指数(PI)、阻力指数(RI)、子宫动脉收缩期血流速度(PSV)水平降低(P<0.05),观察组治疗后子宫内膜厚度及血流动力学相关指标改善优于对照组(P<0.05)。治疗后,2组白细胞介素-2(IL-2)、γ-干扰素(INF-γ)、肿瘤坏死因子-α(TNF-α)水平均较治疗前降低(P<0.05),且观察组治疗后上述指标均低于对照组(P<0.05)。结论:鲜益母草胶囊联合屈螺酮炔雌醇片治疗人工流产术后出血可加快患者子宫内膜功能恢复,降低出血量,抑制炎症发生,临床疗效显著。Objective:To observe the clinical effect of the therapy of Xian Yimucao capsules combined with drospirenone and ethinylestradiol tablets on hemorrhage after artificial abortion.Methods:A total of 86 patients with hemorrhage after artificial abortion were selected and divided into the observation group and the control group,with 43 cases in each group.The control group was treated with drospirenone and ethinylestradiol tablets,and the observation group was treated with Xian Yimucao capsules combined with drospirenone and ethinylestradiol tablets.The clinical effects,menstrual recovery time,vaginal bleeding volume and vaginal bleeding time were compared between the two groups;before and after treatment,the endometrial thickness,indexes of hemodynamics and levels of inflammatory factors were compared between the two groups.Results:After treatment,the total effective rate was 90.70%in the observation group,higher than that of 69.77%in the control group(P<0.05).The vaginal bleeding time and vaginal bleeding volume in the observation group were decreased when compared with those in the control group(P<0.05),and the menstrual recovery time was earlier than that in the control group(P<0.05).After treatment,the endometrial thickness and peripheral blood flow(PBF)in the two groups were increased when compared with those before treatment(P<0.05),and the levels of pulsatility index(PI),resistance index(RI)and peak systolic velocity(PSV)of uterine artery were decreased(P<0.05),after treatment,the improvement of endometrial thickness and indexes of hemodynamics in the observation group was better than that in the control group(P<0.05).After treatment,the levels of interleukin-2(IL-2),interferon-γ(INF-γ)and tumor necrosis factor-α(TNF-α)in the two groups were decreased when compared with those before treatment(P<0.05),and the above three indexes in the observation group were lower than those in the control group(P<0.05).Conclusion:The therapy of Xian Yimucao capsules combined with drospirenone and ethinylestradiol tablets f
