基于MEWS的分级护理干预在CCU老年重症心力衰竭患者中的应用

摘　　要：目的探讨基于改良早期预警评分的分级护理干预在冠心病重症监护病房老年重症心力衰竭患者中的应用效果。方法将92例老年重症心力衰竭患者按照随机数字表法分为研究组47例与对照组45例。两组均给予冠心病重症监护病房常规护理干预,研究组在此基础上给予基于改良早期预警评分的分级护理干预。比较两组冠心病重症监护病房住院时间、机械通气时间、总住院时间、并发症及不良事件发生率,干预前后采用改良早期预警评分、急性生理学与慢性健康状况评分系统Ⅱ评定病情程度。结果研究组冠心病重症监护病房住院时间、机械通气时间、总住院时间显著短于对照组(P<0.01),并发症及不良事件发生率显著低于对照组(P<0.05)。干预后两组改良早期预警评分及急性生理学与慢性健康状况评分系统Ⅱ评分均较干预前显著降低(P<0.01),研究组显著低于对照组(P<0.01)。结论基于改良早期预警评分的分级护理干预有助于改善冠心病重症监护病房老年重症心力衰竭患者病情,促进恢复,降低并发症及不良事件发生率。Objective To investigate the application effect of graded nursing intervention based on modified early warning score(MEWS)in elderly patients with severe heart failure(SHF)in coronary care unit(CCU).Methods A total of 92 elderly patients with SHF were divided into the study group(47 cases)and the control group(45 cases)according to random number table method.Both groups were given routine CCU nursing intervention,and on this basis,the study group was given graded nursing intervention based on MEWS.The length of stay in CCU,mechanical ventilation time,total length of hospital stay,incidence of complications and adverse events were compared between the two groups.Before and after the intervention,MEWS and acute physiology and chronic health evaluationⅡ(APACHEⅡ)were used to assess the severity of the disease.Results The length of stay in CCU,mechanical ventilation time,total length of hospital stay in the study group were significantly shorter than those in the control group(P<0.01).The incidence of complications and adverse events in the study group was significantly lower than that in the control group(P<0.05).After the intervention,MEWS and APACHEⅡin the two groups were significantly lower than those before the intervention(P<0.01),and the study group was significantly lower than the control group(P<0.01).Conclusions The graded nursing intervention based on MEWS can help improve the condition of elderly SHF patients in CCU,promote recovery,and reduce the incidence of complications and adverse events.
