摘  要:目的了解固定无偿献血人群铁营养状况,为进一步完善无偿献血者关爱策略提供支持依据。方法选择2020年3月1日—6月30日在河源市中心血站再次献血的300例固定献血者为研究对象,其中捐献全血总量达到1000、2000、3000 mL和机采血小板5、20、40次的固定无偿献血者各50例。选择同期在本血站进行首次献血的无偿献血者300例为对照组。统计分析各组血清铁(serum iron,SI)和血浆铁蛋白(plasma ferritin,PF)浓度。结果固定无偿献血者和首次无偿献血者PF和SI均值分别为(176.86±80.10)ng/mL vs.(228.03±104.67)ng/mL(t=6.724,P=0.000)、(18.45±4.19)μmol/L vs.(21.36±3.74)μmol/L(t=8.974,P=0.000)。男性和女性固定无偿献血者PF和SI均值分别为(217.91±100.62)ng/mL vs.(69.41±28.70)ng/mL(t=24.582,P=0.000)、(19.36±4.04)μmol/L vs.(16.78±3.88)μmol/L(t=7.978,P=0.000)。机采血小板和全血无偿献血者PF和SI均值分别为(150.21±74.26)ng/mL vs.(218.98±93.89)n g/m L(t=7.036,P=0.000)、(16.78±3.58)μm o l/L vs.(18.73±3.32)μmol/L(t=4.891,P=0.000)。捐献全血总量达1000、2000、3000 mL固定无偿献血者三组PF和SI均值分别为(224.52±115.84)ng/mL vs.(207.33±104.76)ng/mL vs.(125.67±75.92)ng/mL(F=14.067,P=0.000)、(18.69±2.98)μmol/L v s.(19.74±3.53)μmol/L vs.(17.93±2.47)μmol/L(F=4.515,P=0.013)。捐献机采血小板次数达5、20、40次三组固定无偿献血者P F和SI均值分别为(169.27±72.69)ng/mL v s.(145.40±99.12)ng/mL vs.(106.34±83.25)ng/mL(F=6.869,P=0.001)、(17.85±2.99)μmol/L vs.(16.24±3.15)μmol/L vs.(15.37±3.73)μmol/L(F=7.246,P=0.001)。结论采供血机构需要高度重视当地固定无偿献血者铁营养状况,以高频次捐献机采血小板献血者、累计献血量较大的全血献血者以及女性献血者为关注的重点人群。Objective To understand the iron nutrition status of regular voluntary blood donors,and to provide basis support for further improving the health guidance of blood donors.Methods A total of 300 regular voluntary blood donors were selected as the research object who donated 1000,2000 and 3000 mL whole blood and 5,20 and 40 times platelets in Center Blood Station of Heyuan from March 1 to June 30,2020,each 50 cases,and 300 first-time blood donors in Center Blood Station of Heyuan were selected as the control group.The concentrations of serum iron(SI)and plasma ferritin(PF)were statistically analyzed.Results The PF and SI of regular voluntary blood donors and first-time blood donors were(176.86±80.10)ng/mL vs.(228.03±104.67)ng/mL(t=6.724,P=0.000)and(18.45±4.19)μmol/L vs.(21.36±3.74)μmol/L(t=8.974,P=0.000),respectively.PF and SI of male and female regular voluntary blood donors were(217.91±100.62)ng/mL vs.(69.41±28.70)ng/mL(t=24.582,P=0.000)and(19.36±4.04)μmol/L vs.(16.78±3.88)μmol/L(t=7.978,P=0.000).The PF and SI of machine collection platelets and whole blood donors were(150.21±74.266)ng/mL vs.(218.98±93.89)ng/mL(t=7.036,P=0.000)and(16.78±3.58)μmol/L vs.(18.73±3.32)μmol/L(t=4.891,P=0.000),respectively.The PF and SI of the three groups who donated 1000,2000 and 3000 mL whole blood were(224.52±115.84)ng/mL vs.(207.33±104.76)ng/mL vs.(125.67±75.92)ng/mL(F=14.067,P=0.000),(18.69±2.98)μmol/L vs.(19.74±3.53)μmol/L vs.(17.93±2.47)μmol/L(F=4.515,P=0.013).The PF and SI of the three groups of regular voluntary blood donors who donated 5,20 and 40 times platelets were(169.27±72.69)ng/mL vs.(145.40±99.12)ng/mL vs.(106.34±83.25)ng/mL(F=6.869,P=0.001),(17.85±2.99)μmol/L vs.(16.24±3.15)μmol/L vs.(15.37±3.73)μmol/L(F=7.246,P=0.001).Conclusion Blood collection and supply institutions should attach great importance to the iron nutrition status of local regular voluntary blood donors,the key groups of concern are the highfrequency donations of platelet-collected blood donors,whole blood donors with
