多排螺旋CT在儿童气管性支气管诊断中的应用价值

摘　　要：目的 探讨应用多排螺旋CT诊断儿童气管性支气管的价值。方法 收集从2014年1月—2020年6月就诊于河南中医药大学第一附属医院行多排螺旋CT检查的1 436例肺炎患儿,经薄层轴位图像、多平面重组(multi-planar reformation,MPR)、最小密度投影(minimum intensity projection,MinIP)、容积再现(volume rendering,VR)、CT仿真内窥镜(CT virtual endoscopy,CTVE)等处理后,诊断为气管性支气管的29例患者影像资料。结果 29例儿童气管性支气管均开口于气管及右主支气管的右侧壁及右后壁,患儿肺炎右肺均受累,11例累及左肺,9例合并肺气肿,4例有顽固性、反复发作的肺实变。结论 多排螺旋CT具有无创、操作简单,后处理图像可清晰显示气管性支气管的起源部位、狭窄情况及其他合并症,可作为儿童气管性支气管合并肺炎诊断的首选检查方法,为儿童气管性支气管的诊断提供强有力的依据。Objective To explore the value of multi-slice spiral CT in the diagnosis of tracheobronchial in children. Methods Collected 1 436 children with pneumonia who underwent multislice spiral CT examination in The First Affiliated Hospital of Henan University of Traditional Chinese Medicine from January 2014 to June 20 0. After thinsliceaxiali mages,multi-planar reformation(MPR), minimum intensity projection(MinIP), volume rendering(VR) and CT virtual endoscopy(CTVE), the imaging data of 29 patients diagnosed with tracheal bronchus. Results All the 29 children with tracheal bronchus opened in the right wall and right posterior wall of the trachea and the right main bronchus. All the children with pneumonia involved the right lung, 11 patients involved the left lung, 9patients had emphysema, and 4 cases had intractable, recurrent lung consolidation. Conclusion Multi-slice spiral CT is non-invasive and easy to operate, and the post-processing images can clearly display the origin, stenosis and other complications of tracheal bronchus. It can be used as the first choice for the diagnosis of children with tracheobronchial pneumonia, and provides a strong basis for the diagnosis of children with tracheal bronchus.
