基于“阳病治阴”理论探讨腹部推拿在腰椎间盘突出症中的临床应用

摘　　要：目的 观察“阳病治阴”腹部推拿治疗腰椎间盘突出症(lumbar disc herniation,LDH)的临床疗效。方法 将科室2020年1月—2022年6月收治的60例腰椎间盘突出症患者随机分为两组。治疗组30例采用腹部推拿手法治疗,对照组30例采用仰卧位电动牵引法治疗,1次/d,6次/周,连续3周。治疗前后采用日本骨科协会评估治疗分数(Japanese Orthopaedic Association score,JOA)、视觉模拟评分量表(visual analogue scale,VAS)评估疗效。结果 患者腰痛疗效总有效率治疗组为96.67%,高于对照组的73.33%,差异有统计学意义(P <0.05);治疗后,治疗组JOA评分高于对照组,差异有统计学意义(P <0.05);治疗后,两组VAS评分明显下降,差异有统计学意义(P <0.05),且治疗后两组VAS评分差异有统计学意义(P <0.01);治疗组腰腿疼缓解率为80.00%,高于对照组的40.00%,差异有统计学意义(P<0.05)。结论 “阳病治阴”腹部推拿治疗有效减轻腰腿疼及相关症状、体征,改善了腰椎功能,临床疗效优于电动牵引法,值得临床使用。Objective To observe the clinical effect of “Yang disease cure Yin” abdominal massage on lumbar disc herniation(LDH).Methods A total of 60 patients with lumbar disc herniation admitted to the department from January 2020 to June 2022 were randomly divided into two groups. 30 cases in the treatment group were treated with abdominal massage, and 30 cases in the control group were treated with supine electric traction, once a day, 6 times a week, for 3 consecutive weeks. Japanese Orthopaedic Association score(JOA) and visual analogue scale(VAS) were used to evaluate the curative effect before and after treatment. Results The total effective rate of low back pain in the treatment group was 96.67%,which was higher than 73.33% in the control group, and the difference was statistically significant(P < 0.05). After treatment,the JOA score in the treatment group was higher than that in the control group, and the difference was statistically significant(P < 0.05). After treatment, the VAS scores of the two groups were significantly decreased, and the difference was statistically significant(P < 0.05), and the VAS scores of the two groups were significantly different after treatment(P < 0.01). The relief rate of low back and leg pain in the treatment group was 80.00%, which was higher than 40.00% in the control group, and the difference was statistically significant(P < 0.05). Conclusion “Yang disease cure Yin” abdominal massage therapy can effectively relieve waist and leg pain, related symptoms and signs, and improve lumbar spine function. The clinical effect is better than electric traction, and it is worthy of clinical use.
