摘 要:目的:应用能谱CT定量分析煤工尘肺患者肺纤维化中的SiO_(2)/H_(2)O、SiO_(2)/羟脯氨酸(HYP)、SiO_(2)/羟赖氨酸(HYL)含量。方法:搜集已确诊的Ⅲ期煤工尘肺患者30例(尘肺组),选取X线胸片未见明显异常的无接尘史的健康体检人员30例(对照组)。30例尘肺组患者及30例健康受试者均应用GE 128排Revolution能谱CT行胸部CT扫描,扫描范围为双肺尖至肺底,肺容积为4000~6000 cc,应用GSI模式扫描采集数据并进行能谱分析,基于SiO_(2)/H_(2)O、SiO_(2)/HYP及SiO_(2)/HYL基物质配对,测量感兴趣区(ROI)胶原蛋白中的SiO_(2)/H_(2)O、SiO_(2)/HYP、SiO_(2)/HYL含量,提取全肺容积中的SiO_(2)/H_(2)O、SiO_(2)/HYP、SiO_(2)/HYL含量。结果:①尘肺组提取的ROI肺组织内SiO_(2)/H_(2)O基物质含量为(78.95±20.94)mg/cm^(3),对照组为(17.20±6.73)mg/cm^(3),两组差异有统计学意义(P=0.000);尘肺组SiO_(2)/HYP基物质含量为(161.84±17.51)mg/cm^(3),对照组为(30.13±8.95)mg/cm^(3),两组差异有统计学意义(P=0.000);尘肺组SiO_(2)/HYL基物质含量为(173.06±17.23)mg/cm^(3),对照组为(29.99±9.78)mg/cm^(3),两组差异有统计学意义(P=0.000)。②尘肺组提取的全肺组织内SiO_(2)/H_(2)O基物质含量为(12.23±1.56)mg/cm^(3),对照组为(4.94±0.76)mg/cm^(3),两组差异有统计学意义(P=0.000);尘肺组SiO_(2)/HYP基物质含量为(107.16±12.58)mg/cm^(3),对照组为(89.39±4.85)mg/cm^(3),两组差异有统计学意义(P=0.000);尘肺组SiO_(2)/HYL基物质含量为(116.75±10.63)mg/cm^(3),对照组为(97.87±6.24)mg/cm^(3),两组差异有统计学意义(P=0.000)。结论:应用GE 128排Revolution能谱CT可以定量分析煤工尘肺患者肺纤维化中的SiO_(2)/H_(2)O、SiO_(2)/HYP、SiO_(2)/HYL含量,为临床上判断煤工尘肺患者肺纤维化的进程及干预肺纤维化的治疗提供科学客观的影像学依据。Objective:The contents of SiO_(2)/H_(2)O,SiO_(2)/hydroxyproline(HYP),and SiO_(2)/hydroxylysine(HYL)in pulmonary fibrosis patients with coal workers'pneumoconiosis were quantitatively analyzed by GE128 Revolution spectrum CT.Methods:Thirty patients diagnosed with stageⅢcoal workers'pneumoconiosis(coal workers'pneumoconiosis group)were scanned with GE128 Revolution energy spectrum CT,and 30 healthy people with no history of dust exposure and no obvious abnormality on chest X-ray were used as a control group.The scanning range was from the tip to the bottom of the lungs,and the lung volume was 4000~6000cc.GSI mode was used to scan and collect data,and ADW4.7 workstation GSI Viewer was used for energy spectrum analysis.The contents of SiO_(2)/H_(2)O,SiO_(2)/HYP,and SiO_(2)/HYL in the region of interest(ROI)collagen were measured based on SiO_(2)/H_(2)O,SiO_(2)/HYP,and SiO_(2)/HYL base material pairing,and the average value was taken,the contents of SiO_(2)/H_(2)O,SiO_(2)/HYP and SiO_(2)/HYL in whole lung volume were extracted.Results:①Coal workers'pneumoconiosis group of extracting ROI mean SiO_(2)/H_(2)O base material content in lung tissue was(78.95±20.94)mg/cm^(3),control group averages(17.20±6.73)mg/cm^(3),the difference between the two groups have statistical significance(P<0.001).The mean content of SiO_(2)/HYP base substance was(161.84±17.51)mg/cm^(3)in the coal workers'pneumoconiosis group,and(30.13±8.95)mg/cm^(3)in the control group;the difference between the two groups has statistical significance(P<0.001).The mean content of SiO_(2)/HYL base substance was(173.06±17.23)mg/cm^(3)in the coal workers'pneumoconiosis group,and(29.99±9.78)mg/cm^(3)in the control group;the difference between the two groups has statistical significance(P<0.001).②The mean content of SiO_(2)/H_(2)O in whole lung tissue of coal workers'pneumoconiosis group was(12.23±1.56)mg/cm^(3),and that of the control group was(4.94±0.76)mg/cm^(3);the difference between the two groups have statistical significance(P<0.001).The mean co
