摘 要:目的探究DROSHA基因rs10719位点多态性及TORCH感染与育龄女性复发性自然流产(RSM)的关联。方法以2018年12月-2020年12月武汉市第四医院古田院区100例RSM孕龄女性为RSM组,并以同期正常早孕女性72名为对照组,采用聚合酶链式反应-限制性片段长度多态性(PCR-RFLP)检测DROSHA基因多态性,采用酶联免疫吸附法(ELISA)检测TORCH感染及封闭抗体(BA),比较不同流产次数患者、经免疫治疗后BA转阳患者及BA阴性组的DROSHA基因多态性及TORCH感染率,分析其对经免疫治疗后BA转阳的预测价值。结果RSM组DROSHA基因rs10719位点TT/TC型检出率及TORCH感染率高于对照组,DROSHA基因rs10719位点多态性及TORCH感染是影响RSM发生的危险因素(P<0.05);TORCH感染组及未感染组DROSHA基因rs10719位点多态性对比,无统计学差异;不同流产次数患者的DROSHA基因多态性及TORCH感染率对比,无统计学差异;BA转阳及BA阴性组的DROSHA基因多态性对比,无统计学差异;BA转阳组的TORCH感染率低于BA阴性组(P<0.05);TORCH感染预测经免疫治疗后BA转阳的AUC为0.757。结论DROSHA基因rs10719位点多态性及TORCH感染是影响反复性自然流产发生的危险因素,但与复发流产次数无关,且TORCH感染对RSM患者经免疫治疗后BA转阳具有预测价值。OBJECTIVE To explore the association between the gene polymorphisms at rs10719 locus of DROSHA gene,TORCH infection and the recurrent spontaneous miscarriage(RSM)in women of childbearing age.METHODS A total of 100 women of childbearing age with RSM who were treated in Gutian District of the Fourth Hospital of Wuhan from Dec 2018 to Dec 2020 were assigned as the RSM group,meanwhile,72 healthy women with early pregnancy were chosen as the control group.The DROSHA gene polymorphisms were detected by using polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP),the TORCH infection and blocking antibody(BA)were detected by means of enzyme-linked immunosorbent assay(ELISA).The DROSHA gene polymorphisms and incidence of TORCH infection were compared among the patients with different number of times of abortion and betwee RESULTS The detection rate of TT/TC genotype at rs10719 locus of DROSHA gene and incidence of TORCH infection were significantly higher in the RSM group than in the control group,the polymorphisms at rs10719 locus of DROSHA gene and TORCH infection were the risk factors for RSM(P<0.05).There were no significant differences in the polymorphisms at rs10719 locus of DROSHA gene between the TORCH infection group and the no infection group.There were no significant differences in the polymorphisms of DROSHA gene and incidence of TORCH infection among the patients with different number of times of abortion.There were no significant differences in the polymorphisms of DROSHA gene between the BA-positive group and the BA-negative group.The incidence of TORCH infection of the BA-positive group was significantly lower than that of the BA-negative group(P<0.05).The AUC of TORCH infection was 0.757 in prediction of positive conversion of BA after immunotherapy.CONCLUSION The polymorphisms at rs10719 locus of DROSHA gene and TORCH infection are the risk factors for RSM but were not associated with the number of times of abortion.The TORCH infection had the value in prediction of positive convers
